I. Objective: You are going to create a brochure entitled “Healthy-e Lifestyle”.
II. Materials:
Colored or bond paper Coloring materials Ruler Cut-out pictures
III. Procedure:
1. Fold your paper into 3 parts.
2. Make the first panel of your brochure the title page.
3. Once done, you are going to create a brochure about “Healthy-e Lifestyle”. Read the instructions
given per subject for you to come up with the desired output for this integrative performance task.
MAPEH:
On the 2nd column, paste 3 to 5 pictures of Related Fitness/Physical activities that
can help you and your family stay healthy despite these pandemic days. You may
research through the internet or cut pictures from old magazines. Write a
description on how it will help you and your family stay healthy.
English: On the 3rd column, write an essay (minimum of 5 sentences) narrating your physical fitness
experience by answering the following questions.
1. What are the tools you used in your physical fitness activities? Enumerate at least 5 and briefly tell
their uses.
2. Explain how you performed the physical fitness activities.
3. From your essay, find the nouns, modal verbs and adverbs used and fill out the table below:

MATH At the back page (1st column), make a 5-month weight/loss target plan with a constant loss/gain.
(You may use the weight/loss log below). Using the coordinates (month #, weight), plot the points on the
cartesian plane and determine the slope of the line using any two points on the line.

1. What will happen to the temperature of a person if he/she is

TLE At the back page (3rd column), answer the following guide questions:
1. Give 5 hazards and risks that may happen to you while doing an exercise.
2. How can you avoid these hazards and risks?
