SKILL: Nebulization

Nebulization is a form of respiratory care. It is the medical process of administering


medication directly by inhalation with the help of a nebulizer that converts liquid
medicine into mist which is given to the patient.
STEPS
1. Review medical orders and obtain information on drugs/dosage/specifics of
administration/ and other medical and nursing theory. Rationale: This ensures
accurate handling and precise administration of drugs to the right patient.

The doctor’s order is to administer Albuterol Aerosol Nebulizer Treatment one dose
of 2.5 mg in 3ml of normal saline.
2. Explain to client the procedure and purpose. Rationale: To ensure cooperation
and effectiveness of the treatment.

Hi Ms. Julie, as administered by your physician, I will be performing nebulization. This


is a good respiratory therapy that converts liquid medicine into mist for inhalation.
3. Prepare instrument and equipment needed.
 Prescribed drugs
 Patient’s medical records/chart
 Air compressor/nebulizer
Rationale: This helps save time and promotes error free administration.
4. Place the patient in a comfortable sitting semi-Fowler’s position. Rationale: To
ensure maximal distribution and deposition of aerosolized particles at the base of
the lungs.
5. Add the prescribed amount of medication and saline solution to the nebulizer.
Rationale: To enable deep penetration into the tracheobronchial tree.
6. Connect the tubing to the compressor and turn it on. Rationale: To notice the mist
produced by the nebulizer.
7. Instruct client to exhale. Rationale: This is for preparation to start.

Instruct the client to grip the mouthpiece with the lips and inhale as slowly and deeply
as possible through the mouth.
8. Observe expansion of chest to ascertain that patient is taking a deep breathe.
Rationale: To ensure that the medication is deposited below the oropharynx.
9. Instruct the client to breathe slowly and deeply until medication is nebulized.
Rationale: This ensures effective administration of the medicine.
10. On completion of the treatment, encourage client to cough after several breaths
deeply. Rationale: To facilitate expectoration of secretion.
11. Assess need for percussion, vibration, postural drainage and suctioning.
Rationale: These processes mobilize accumulated secretion and unclog the
airways to keeping lungs healthy.
12. Record medication used and description of secretion
(if any). Rationale: Proper documentation is a proof of work done and observed.
The medicine administered was Albuterol Aerosol Nebulizer Treatment one dose of
2.5 mg in 3ml of normal saline. No signs of respiratory secretions are noted.
13. Disassemble and clean nebulizer after each. Rationale: Proper cleaning,
sterilization and storage of equipment prevent organisms from entering the lungs.
