Dear Mr. and Ms. Javizian’
As you know, | am the Chief Medical Examiner for Wayne County. The situation of our
office not timely ascertaining the identity of your son Gregory and contacting you came to my
attention soon after that error was discovered. The situation was promptly addressed with the
investigator involved and corrective action taken. In addition to our condolences on the loss of
your son, you have our sincere apology for our office not taking the proper steps to notify you in
a timely manner.
Asa result of this very rare, but serious and unfortunate incident with your son, the Chie
Investigator has instituted certain changes to try and ensure this kind of error is not repeated. In
particular, in response to this incident and your expressed concerns, the Chief Investigator runs
weekly reports regarding open cases/tasks, and cross-references these with the investigators
assigned to follow up. Thus, for example, if there were an open task regarding notification of
next of kin, follow up action could be immediately taken to ascertain the reason for any delay
You should also know that we are working with our computer vendor to merge the two reports,
such that the cross-referencing will occur automatically in a single report. Also, as of fall of
2017, the date a case is released is also printed along with the case number in the daily list of
cases; at a glance, you can look at the cases for a particular day and know which cases are still in
storage and which have been released.
I recognize that no words can really ease the pain of the loss of a loved one, But I hope
you know that we quickly took your concerns to heart and instituted these changes when we
identified this potential gap in our process.
