abdomen. Educate patient’s mother on
distraction techniques such as guided
imagery.

R- Seen patient cuddled by mother and sleeping.

9:00 am Fever D- Temperature: 37.8°C, skin is warm to touch,


flushed face noted.

A- Performed tepid sponge bath, and encourage


increase in fluid intake. Offer extra blankets.

R- Decrease in temperature of 37.5 °C.
