REQUEST FOR PROPOSAL

NEGOTIATED PROCUREMENT THROUGH EMERGENCY CASES FOR THE PROPOSED 2-STOREY


ISOLATION BUILDING

Resolution No. _11-0443__, 2020

1. The Davao Regional Medical Center (DRMC), as a Government Procuring Entity, has identified the
requirement for the Negotiated Procurement through Emergency Cases for the Proposed 2-Storey
Isolation Building.

2. A complete set of bidding documents including the Program of Works, Bill of Quantities, and
other Terms and Conditions may be purchased by interested bidders on November 23-27,
2020from the address below and upon payment of a non-refundable fee for the bidding
documents in the amount of

3. Proposal must be delivered at the PROCUREMENT SECTION/ BAC SECRETARIAT, DRMC, Apokon,
Tagum City or through Electronic Mail at on or before
November 27, 2020, 2:00 P.M.

4. Your price quotation must be quoted in Philippine Peso and both in word and in figure in
computerized/typewritten inclusive of all taxes to be incurred.

5. All documents listed below shall be fastened in a red folder enclosed in a brown envelope with all
pages signed for authenticity.

a. Original/Certified true copy of Detailed estimates, including a


summary sheet indicating the unit prices of construction materials, labor
rates, and equipment rentals used in coming up with the Bid.
b. Original/Certified true copy of Valid PhilGEPS Registration Certificate
(Platinum Membership)
c. Original/Certified true copy
Permit/ License issued by the city or municipality.
d. Original/Certified true copy of Statement of the prospective bidder of
all its on-going government and private contracts, including contracts
awarded but not yet started, if any, whether similar or not similar in
nature and complexity to the contract to be bid. The statement shall include,
for each contract, the following:
i. name of the contract;
ii. date of the contract;
iii. contract duration;
iv.
v. nature of work;
vi.
percentage of participation;
vii. total contract value at award;
viii. date of completion or estimated completion time;
ix. total contract value at completion, if applicable;
x. percentages of planned and actual accomplishments, if applicable;
xi. value of outstanding works, if applicable;
e.
similar to the contract to be bid, except under conditions provided under
the rules.

The prospective bidder must have completed, within the period specified
in the Invitation to Bid, an SLCC that is similar to the contract to be bid,
and whose value, adjusted to current prices using the Philippine Statistics
Authority (PSA) consumer price indices, must be at least fifty percent
(50%) of the ABC. However, in the case of Expendable Supplies, said
SLCC must be at least twenty five percent (25%) of the ABC.

If, at the outset and after conducting market research, the Procuring
Entity can already determine that imposing the same will likely result to:
(a) failure of bidding, or (b) monopoly that will defeat the purpose of
competitive bidding, the Procuring Entity, in lieu of the above, may
require the following:

The prospective bidder should have completed at least two


(2) similar contracts and the aggregate contract amounts
should be equivalent to at least the percentage of the ABC as
required above; and

b) The largest of these similar contracts must be equivalent


to at least half of the percentage of the ABC as required
above.

The statement shall include, for each contract, the


following:
i. name of the contract;
ii. date of the contract;
iii. contract duration;
iv.
v. kinds of Goods;
vi. amount of contract and value of outstanding contracts;
vii. date of delivery;
viii. the statement shall be supported by the Purchase Order or
Contract Agreement or NOA stating the amount of the project;

and/or notices to proceed issued by the owners; and

of Final Acceptance or the Certificate of Completion and,

Evaluation Summary (CPES) Final Rating, which must be satisfactory.

f. Original/Certified true copy of Valid Philippine Contractors


Accreditation Board (PCAB) license
and registration applicable for Government Projects Or Special PCAB
License in case of Joint Ventures

g. Original/Certified true copy of DULY SIGNED Project Requirements, which shall


include the following:
i. Organizational chart for the contract to be bid;
ii.
Materials Engineers, and Foremen) to be assigned to the contract to be Bid,
with their complete qualification and experience data, with Curriculum Vitae
including copy of updated licenses or proof of renewal of their licenses, if
expired. The list must contain the name, educational attainment, training,
professional license and work experience of personnel. To wit:

purchase agreements, supported by proof of ownership or Certification of


availability of equipment from the equipment lessor/vendor for the duration of
the project, as the case maybe;
iv. Construction schedule and S-Curve;
v. Manpower Schedule;
vi. Construction Method in narrative form;
vii. Equipment Utilization Schedule;
viii. Construction Safety and Health Program to be approved by the Dept. of
Labor & Employment
ix. PERT/CPM;
x. Affidavit of Site Inspection;
xi. Plans (Blueprint) ( ); and,
xii. Summary of Works.

h. Original/Certified true copy of OMNIBUS SWORN STATEMENT (OSS) with


Secretary Certificate (SC) or Special Power of Attorney (SPA)

i. Original/Certified true copy of Audited Financial Statements (AFS),

Revenue (BIR) or its duly accredited and authorized institutions, for the
preceding calendar year which should not be earlier than two (2) years
( ) from the date of bid submission:

i. Balance Sheet
ii. Income Statement

. However, in the case of procurement of
Goods, a bidder may submit a committed Line of Credit from a Universal
or Commercial Bank, in lieu of its NFCC computation.

NFCC = [(Current assets minus current liabilities) (15)] minus the


value of all outstanding or uncompleted portions of the projects
under on-going contracts, including awarded contracts yet to be
started, coinciding with the contract to be bid.

NFCC computation must be at least equal to the Approved

assets and current liabilities shall be based on the data submitted


to the BIR, through its Electronic Filing and Payment System
(EFPS).
If the prospective bidder submits a it must
be at least to ten (10%) of the ABC to be bid: Provided, that if the
same is issued by a foreign Universal or Commercial Bank, it shall be
confirmed or authenticated by a local Universal or Commercial Bank.
