I. Pre-Entry Phase
Learning Objectives
Initial phase of the organizing process where the community

V. Exit Phase/ Phase Out
It is when the health workers leave the community to stand alone. This
phase should be stated during the entry phase so that people will be
ready
The organization build should be ready to sustain the test of the
community itself because the real evaluation will be done by the
residents of the community itself
