    We offer our partnership with you in ensuring your success in Senior High School level and be ready
    for your College/University goals. This academic success plan hopes to strategize the challenges
    that you will meet along your stay with us.

    This plan is not confidential and is meant to be a collaborative effort between you and your Adviser,
    Subject Teachers, the Senior High School Guidance Office and Strand Coordinator.

    Academic Success Plan Requirements:

    1. Regularly attend the Daily check-in/Homeroom Guidance Period


    2. Connect regularly at least once or two times every week with your Adviser.
    2. Check the attendance policy of the school. Attend all classes regularly on time.
    Three absences in one course/subject is equivalent to one absence. You should
    not exceed the allowed number of excused absence within a semester.
    3. Monitor your grades.
    4. Submit all class assignments /activities on time.
    5. Achieve at least 80 or higher average for this semester.
    6. Seek your adviser’s or subject teacher’s academic assistance on any difficulty
    and make an appointment during the academic consultation hour.

    very best to keep all the requirements stated herein.
