HOME ISOLATION FOR COVID CARE

Women’s Health Collective

The atmosphere all around is charged with fear and anxiety, not just
because of the 24/7 news on the TV sets but information pouring in from
far and wide about the lives lost and desperate family members seeking
medical care in terms of Beds, Oxygen and medicines.

The idea that one can "avoid" getting infected by SARS-CoV2 (by all its
variants) for long, just by being locked up, is futile. Sooner or later, most
of us will come into contact (if not already). So best is to be prepared,
boost your immunity and get equipped (details below)

With proactive measures, we can significantly reduce the impacts /


degree of illness, even if infected by the virus.
- Panic (and consequent frantic & incoherent actions) is actually
detrimental to your response. Plan ahead, be calm, get necessary
things ASAP (details below).

So here we go armed with information which will help you overcome the
actual virus and the fear and anxiety associated with it.

What is Covid?

-It is a viral infection transmitted from one infected person to another


healthy individual through the air we breathe.

-After it passes through the nasal passages, the virus has the ability to
infect the lung and cause pneumonia, and over time destroys the lung
tissue causing breathlessness .

This infection can be roughly divided into 3 phases…of course there are
overlaps

Phase-1 Viraemic phase (5-7 days) When the virus is multiplying in the
body of the infected person, and easily spreads to others in the family or
people in close vicinity specially if they are not protected with masks.

Phase-2 Pulmonary phase (5-10 days) When the virus enters the lungs,
infects it and spreads rapidly causing pneumonia.

Phase-3 Post inflammatory response phase (10-21 days) called a

Cytokine Storm, here the body launches an immune attack against the
virus but ends up damaging the lungs instead. The badly affected lungs
are unable to pick up oxygen leading to hypoxia and oxygen deprivation.

Many of us have symptoms of COVID but the RTPCR may or may not
be positive.
If you have symptoms it is now treated like Covid.

If you or anyone you know has Fever, or Cough, or Loose motions, or


Loss of smell and taste, or General fatigue … accompanied by other
flue like symptoms like headache, body ache, running nose, sore throat
etc. DO NOT IGNORE IT …

You can get yourself a RT PCR test. But remember it takes a long time
to arrive. Don’t keep waiting for the result to come. Instead treat the
condition as COVID and follow the protocol given here.

As with any flue, Most of us… and we Mean most of us.. will recover in
5-7 days.

But this flue is infectious, spreads like wildfire that it demands that you
are isolated from the senior citizens in your vicinity and others who may
not be as lucky as you.

You can also follow any other Protocol circulated by


the Government / Health bodies like the AIIMS,
New Delhi, or one made by the Indian Medical
Association (IMA) or Indian Academy of Pediatrics
(IAP)
They are not very different

So if you have any of the above symptoms even without a positive report
please Isolate yourself at Home

I. Home Isolation -Anyone with suspected or very mild symptoms


(and those in contact) should take necessary precautionary steps
like isolation, distancing, mask use, not sharing utensils & toilets etc;
(See Annex.1)

I.a. GENERAL

a) Rest… as in actual rest…this is the time to catch up with friends on


Whatsapp, phone, listen to music or watch movies. You can sit
down and write if that helps.
b) Keep a record of everything you do or feel. For Example note
down symptoms like nausea, fatigue, or any other, also foods you
have consumed. This will be useful if you need to consult a doctor
later
c) Monitor temperature 2-3 times a day
d) Measure Oxygen saturation 2-3 times a day. If it is above 94%
don't worry
e) Drink plenty of warm water or any warm liquids like tea, soups,
Fresh fruit juices, buttermilk, and coconut water to be taken at
room temperature, throughout the day…or every one hour to flush
out the virus
f) Keep eating food, to beat the fatigue unless you are throwing up
g) If you can, sit in an open balcony or any open space and expose
yourself to the sun for 15-30mins between 11AM and 3PM
everyday .

I.b MEDICATION

1) Ivermectin 12 mg once a day for 3 days on an empty stomach


(According to new guidelines)
2) Dolo 650mg three times a day if the fever is more than 100*F OR
37.8*C
3) Steam inhalation 3 times a day, up to 8-10mins. each time
4) Betadine gargling- dilute Betadine in warm water and gargle 2-3
times a day
5) Vitamin C 500mg. tabs 2 times a day
6) Zincovit tab once a day
7) Vitamin D 60,000 IU once a week for 4 weeks and then once in 15
days
8) Doxy 100mg 2 times a day, for 5 days, or your doctor may give you
Azithromycin, once a day for 5-7 days.
9) Cetirizine Tab If there is throat irritation, sneezing or running nose:
T (Zyrtec): One tab once a day .
10) Budesonide Inhalational (given via inhalers with spacer at a dose
of 800 mcg twice daily for 5 to 7 days) to be given IF symptoms
(fever and/or cough) are persistent beyond 5 days of disease onset
11) ORS for loose motions. You may also Sporolac/Econorm

twice a day before food.
12) Pantoprazole 40 one tab. in the morning on a empty stomach
13) Those with co-morbidities such as obesity, diabetes, high BP,
asthma, kidney disease, smokers, chronic chest diseases, history
of lung tuberculosis,or other chronic problem like thyroid etc.
Please do not stop those medications, continue with your doctor’s
advise.
In addition people with co morbidities
May be advised to do the following tests by their doctor
CRP, Serum Ferritin, LDH, D-Dimer, Interleukin 6.

NOTE-1 If you have been using homeopathic medicine please get


your homeopathic doctor to prescribe for the above condition. But
use Allopathic medicines too.

NOTE-2- Injectable steroids are not indicated in mild disease. IF


symptoms persist beyond 3-5 days (persistent fever, worsening
cough etc.) consult the treating doctor for treatment with oral or IV
steroids.

NOTE-3 The decision to administer Remdesivir or any other


investigational therapy must be taken by a medical professional
and administered only in a hospital setting. Do not attempt to
procure or administer Remdesivir at home. So tell your family
not to hunt for this medicine. In any case it has very limited
use.

NOTE-4 Do not try to stock up on Oxygen cylinders, Oxygen has


to be administered with medications like steroids. Use of
unsupervised Oxygen will mask a deteriorating lung condition

II. When to discontinue home isolation

Patient under home isolation can end isolation after at least 10 days have
passed from onset of symptoms (or from date of sampling for
asymptomatic cases) and no fever for 3 days. There is no need for
testing after the home isolation period is over.

III. When should you seek medical attention ie hospitalization

As you monitor your parameters and symptoms and if you notice
that
By the 3-5th day the fever is high, your throat has got worse, you are
coughing much more, and all the other symptoms continue or get worse,
In addition if there is
壱 i. Difficulty in breathing,
ii Dip in oxygen saturation (SpO2 < 94% on room air)
iii. Persistent pain/pressure in the chest,
iv. Mental confusion or inability to arouse,

You will have to see a doctor. Also get the following tests which are
important for baseline assessment.

1) Oxygen Saturation with Oximetry


2) Pulse rate
3) Blood Pressure
4) Blood Sugar
5) Complete Blood Picture
6) CRP
6) LFT
7) Serum Creatinine
8) HRCT of the lungs

弐 Immediate medical attention must be sought


IF any one of the following parameters are affected.

1. High Fever, cough, diarrhoea continues


2. High CRP levels
3. Oxygen saturation of less than 94
4. CT severity score suggestive of lung involvement (more than 8)
with or without chest pain
5. Co morbidities like obesity, diabetes etc.

You need Steroids, (oral or injectable) and oxygen under a doctors


supervision. In addition blood thinners may be given.

The clinical condition and the reports help the doctor decide whether
home treatment can be continued with Steroids or you require
admission in the hospital.

TREATMENT_ The only known treatment for Covid
is
Steroids and Oxygen,
but these must be used after 3-5days if parameters
deteriorate

Some young patients aged 25-45yrs have low


oxygen, high CRP levels and lung damage(CT
Score >8) on day one, they need Steroids and
Oxygen on arrival

How to recognize low oxygen saturation when a


Pulse Oximeter is not available,

Proxy clinical signs of respiratory compromise and


poor Oxygen Concentration assessed by
noting
1) Effort needed to breathe
2) Ability to talk- even on phone is difficult
3) Low hand grip
4) Persistent weaker cough

How to improve your oxygen saturation


1. For improving your oxygen saturation sleep on
your stomach with a pillow below your hips, and
place your face on a thin pillow such that your
chest is not compressed and breath normally

2. Sit up straight and do deep breathing exercises

3. Try to blow a large balloon in one go

Annexure-1

Home Isolation

*Strict isolation* at home. All positive patients can stay in the same house
and do not have to remain in separate rooms.

i. Patient must isolate himself from other household members, stay in


the identified room and away from other people in home, who are not
infected especially elderly and those with co-morbid conditions like
hypertension, cardiovascular disease, renal disease etc.

ii. The patient should be kept in a well-ventilated room with cross


ventilation and windows should be kept open to allow fresh air to come
in.

iii. Patient should at all times use triple layer medical mask. Discard
mask after 8 hours of use or earlier if they become wet or visibly soiled.
In the event of care giver entering the room, both care giver and patient
may consider using N 95 mask.

iv. Mask should be discarded only after disinfecting it with 1% Sodium


Hypochlorite.

v. Patient must take rest and drink lot of fluids to maintain adequate
hydration.

vi. Follow respiratory etiquettes at all times.

vii. Frequent hand washing with soap and water for at least 40 seconds
or clean with alcohol-based sanitizer.

viii. Do not share personal items like towels, bedsheets, combs etc. with
other people in the household_

ix. Ensure cleaning of surfaces in the room that are touched often
(tabletops, doorknobs, handles, etc.) with 1% hypochlorite solution.
x. Self-monitoring of blood oxygen saturation with a pulse oximeter is
strongly advised.

xi. The patient will self-monitor his/her health with daily


temperature
