    AFFIDAVIT
    I, XXXX, of legal age, single, Filipino and a resident of #91 San Nicolas Betis, Guagua,
    Pampanga, after having been duly sworn to in accordance with law, depose and say:

    1. That I am the legal holder of a Philippine Passport bearing Number XXXX issued by the
    Department of Foreign Affairs (DFA for brevity) Angeles, Pampanga valid until
    April 21, 2022, a photocopy of which is attached hereto as Annex “A” and made an
    integral part hereof ;

    2. That, in the said passport, it is evident in my picture is my MOLE on my left face near
    the lips and just below my left cheek ;

    3. That when I worked in Dubai in 2017, in my work as a SPA attendant, my employer


    required me to remove my MOLE and I had such mole removed thru laser removal and
    now the mole is not apparent on my face ;

    4. That at this juncture, I do vouch and warrant that I am the same person as appearing in
    my Philippine Passport bearing Number XXXX and the same person that is now
    applying for the renewal since the other mole in the right face which was not removed is
    still in my face as it is in my face on the time I had my passport and it can be ascertained
    that I am the same person ;

    5. That this Affidavit is executed in order to attest to the veracity of the foregoing
    statements and in compliance with the requirements of Department of Foreign Affairs
    and for any and all legal purposes it may serve.

    FURTHER AFFIANTS SAY NOT.

    Philippines. Affiant is personally known to me and for proper identification he presented to me
    his Philippine Passport No. XXXX with his picture thereon and he signed the above affidavit in
    my presence.
