    AFFIDAVIT OF LATE FILING OF SALN

    I, MSG RANIEL T. TUBIANOSA 793376 PA, of legal age Filipino, with residence address at
    # 1 L. Pasco St., Corner Doña Yayang, Brgy. Libis, Quezon City, after having sworn to in
    accordance with law do hereby deposes and state:

    1. That I am an active member of AFP with unit assignment at AFPHSC, dy a/u HHSG with
    office address V. Luna Road, Quezon City;

    2. That this pertains to my Statement of Assets, Liability, and Net Worth (SALN) as of 31
    December 2019 and 31 December 2020 that I am required to file within the prescribed
    period, but due to multiple health problems, where I was confined twice, the same
    however were belatedly file (please see attached Medical Certificates).

    3. Considering the above factual circumstances, the failure to file on time the two SALN
    were unintentional and were beyond my control, hence, I respectfully request with the
    concerned Office that the belatedly filing of my said SALN for 31 December 2019 AND 31
    December 2020 be justified.

    4. That I am executing this affidavit to attest to the truth of the foregoing instrument and
    for any legal purpose and intents this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 1 st day of June, 2021 in Quezon
    City.
