POST ACTIVITY REPORT (OR TERMINAL REPORT)

I. TITLE OF THE ACTIVITY :DONTOGAN BARANGAY ANNUAL


PAPSMEAR

II. DATE :25 September 2019

III. HIGHLIGHTS OF THE : Collection of cervical specimen, assessment


ACTIVITY and educating them regarding pap smear test
and its effectiveness in cervical cancer detection.
Last September 25, 2019 the Community Health Team of Dontogan Barangay conduct a free pap
smear among women on barangay patients ages 21 to 65 yrs. Old. Were included.
All the women were sensitized about the screening method to detect carcinoma of the cervix in
preclinical stage. The women who volunteered to participate were reinformed about the Pap
smear, biopsy if required, and the required follow-up in case of an abnormal Pap test results with
the help of an information sheet that was provided to them and all queries were answered by the
investigators. Thereafter, informed consent was obtained. A detailed history was taken in all the
women and that included personal information, history, and clinical examination. It was ensured
that no local douche, antiseptic cream, and no local internal examination were done on the day of
test. The patient was placed in lithotomy position, a Cusco's bivalve speculum was introduced
through the vagina, and the cervix was visualized.

The Barangay Community Health Team—Kag. Presciosa Esteban, Midwife Remy Poclan, BNS
Betsy Pablo, Julie Tiago and BHW Cherry Ann Ferrer. Conducting assessment and educating the
women and made aware about cervix cancer and importance of early detection by regular
screening program as prevention is better than cure.

A. Eco-Walk Going to Mount Cabuyao – 11 July 2015

A total of ______ adults and children from the barangay joined the Eco-Walk to Mount Cabuyao,
They enjoyed the scenic view and fresh air going up the mountain and going back to the
barangay. A brief lecture on the importance of walking regularly to one’s health was also given
by Ms. _________ .

B. Day Care Health and Nutrition Class – 13 to 16 July 2015

The 50 students of Barangay Gomburza Day Care Center have undergone a 4-day Health and
Nutrition Class. Ms. Brenda Castro, the Barangay Nutrition Scholar, facilitated the class. She
explained and demonstrated the following topics:
1. Proper Hand Washing
2. Proper Toothbrushing
3. Importance of Regular Dental Check-up

C. Mass Feeding – 22 July 2015

A total of 30 school children participated in the mass feeding this year as part of the Celebration.
The parents joined the barangay volunteers in preparing the nutritious food for their children.
BNS Gemma Brion gave a short lecture on the importance of preparing good food for the young
before the mass feeding.

D. Mother’s Class – 1,8,15,21,28 July 2015

The 15 mothers successfully completed the Mother’s Class this year. BNS Gemma Brion
facilitated the activity held at the barangay multi-purpose hall. The topics discussed during the
5-day class were the following:

E. Distribution of Vegetables – 22 July 2015

A total of 100 vegetable seedlings were distributed to 50 families of the barangay. Before the
seedlings were distributed, BNS Brion discussed the proper way on how to take care of the given
seedling and why is it given to them as part of Barangay Gomburza’s Nutrition Program. She
emphasized that backyard gardening is encouraged to ensure that organic vegetables are readily
available for the consumption of the family.

F. Culminating and Awarding Program – 31 July 2015

1. Preliminaries

Barangay Kagawad Liza Santos led the opening prayer during the Culminating and Awarding
Program while Barangay Secretary Cita Linas led the singing of the Philippine National
Anthem.

2. Welcome Remarks

PB Ong warmly welcomed all the barangay constituents. He also thanked the guests for
taking the time to join the barangay for the 2015 Nutrition Month Celebration.

3. Introduction of the Guest Speaker

BNS Gemma Brion gave a brief introduction of Mr. Joel Junsay from the City Nutrition Office
as the guest speaker for this year’s July Nutrition Month Celebration.

4. Message

The guest speaker, Mr. Joel Junsay, focused his message on the importance of exercise,
proper nutrition and maintaining the ideal weight of an individual.

5. Awarding of Winners

The winners for the various contests conducted as part of the Nutrition Month Celebration
were awarded. PB Ong and the Barangay Officials led the awarding.

Barangay Kagawad Joel Dugang gave the closing remarks. He thanked all the barangay
constituents and guests. Kagawad Dugang also thanked the BNS, BNAO, the members of
the Barangay Nutrition Council and all the efforts of the Barangay Officials for making the
celebration a success.

by Dr. Tubera for a dengue-free barangay.

TITLE OF THE ACTIVITY : OPT PLUS 2015

DATE : January to December 2015

HIGHLIGHTS OF THE ACTIVITY

1. General Weighing

This was conducted on February 4, 2015 where 125 0-71 months children of the barangay were
weighed. The Barangay Nutrition Scholar together with the members of the Barangay Nutrition
Committee went to all the puroks to weigh the children. They were assisted by the five (5)
Barangay Tanods. Among the 125 children, 15 found out to be underweight. Of the 15 children,
five (5) were 0-24 months and 10 were 25-71.

2. Monthly monitoring of underweight children aged 0-24

The weighing of the five (5) 0-24 months underweight children was done every 1 st Saturday of
the month. The BNS was assisted by the members of the BNC and the barangay tanods. The
families of the children were cooperative and there were no problems encountered during the
monitoring. The mothers were advised to feed their children with nutritious foods. IEC
materials were also given to them. Case studies were prepared for Babies C, D and E and
interventions for each was made as contained in the study. Below is the monthly weights (in
kilos) of the five (5) children:

At the end of the year, all the babies weights were within the normal range. (if not all, you can
State it as: At the end of the year, weights of Babies A,B,C,D were within the normal range. On
the other hand, Baby E will still be monitored on a monthly basis.) State the reason why Baby E
remained underweight.

3. Quarterly monitoring of underweight children aged 25-71 months

The weighing of the 10 25-71 months underweight children was done every 2 nd Saturday of the
first month of the quarter. The BNS was assisted by the members of the BNC and the barangay
tanods. Some parents of the children were not cooperative during the monitoring. There were
mothers who were advised to feed their children with nutritious foods but failed to follow. IEC
materials were given to them. Each child was prepared a case study. Below is the quarterly
weights (in kilos) of the 10 children:

At the end of year, the weights of Child A,B,C,D,E and F were within normal range while Child
G,H,I, and J who are still underweight will be monitored continuously on a monthly basis. State
reason why Child G,H,I,J remained underweight.
