    We, HELEN GRACE F. FENEQUITO and NERIO A. FENEQUITO,


    both of legal age, Filipinos, married, and residents of Purok Paghidait, Busok,
    Bagumbayan, Sultan Kudarat, after having been duly sworn to in accordance with
    law, depose and say:
     
    1. That we are the parents of NERALYN FUNA FERMOCIL, who was
    born on 10 January 1998;
    2. That we were not yet married at the time NERALYN FUNA
    FERMOCIL was born;
    3. That we subsequently got married on July 31, 1999;
    4. That we have no legal impediment to marry each other from the time of
    conception of NERALYN FUNA FERMOCIL;
    5. That due to our subsequent marriage, we would like to legitimize our
    daughter NERALYN FUNA FERMOCIL;
    6. That we are executing this affidavit to attest to the veracity and
    truthfulness of all the foregoing facts and circumstances and for the
    purpose of legitimation of NERALYN FUNA FERMOCIL.

    March 2019 in Isulan, Sultan Kudarat.

    of all the foregoing facts and circumstances and for whatever purpose
    this may serve best.

    accordance with law, depose and say:
     
    1. That I am the mother of BRENT KYZER SUAREZ BANDAO, who
    was born on June 24, 2020 in Isulan, Sultan Kudarat;

    2. That he was not registered because we waited for his father to sign his
    birth certificate but he was affected by the lockdown in Iligan City;

    3. That I and his father are not married;

    4. That his father was now able to sign his birth certificate hence, we are
    now registering his birth;

    5. That I am executing this affidavit to attest to the veracity and truthfulness


    of all the foregoing facts and circumstances and for the purpose of
    registering the birth of my son BRENT KYZER SUAREZ BANDAO.
