Cancellation of Previously Approved Survey Plans

Requirements:

1. Letter request from private party (registered owner/government Agency stating the reason
for cancellation. (Notarized)
• If the requestee are Heirs, attach Extra Judicial settlement among Heirs,
• If the requestee is Authorized Representative, attach Special Power of Attorney
• If the requestee is a Corporation, attach Secretary’s certificate authorizing the person
who represent the company.

2. Updated certified copy of title of the subject lot/lots


3. Sepia copy of approved plan/Affidavit of Loss in case of lost sepia copy
4. Copy of new subdivision plan if there’s any
5. Certified blue print copy of approved plan to be cancelled

For untitled Property:


1. Endorsement from CENRO/PENRO requesting/recommending the cancellation of untitled
Lot/property
2. Investigation Report/Ground verification if there is a valid reason for cancellation.
