AFFIDAVIT OF CONFIRMATION OF LOSS

I, REMEDIOS DEL MUNDO TAGARRO, of legal age, single, Filipino citizen and a
resident of Natalna Cpd., Pagudpud, San Fernando City, La Union, after being sworn
in accordance with law, hereby depose and state that:
1) I am the Attorney-in-Fact of one, RUFINA N. PINZON;
2) I was entrusted by Rufina N. Pinzon the owner’s duplicate copy of Transfer

Certificate of Title No. T-26403 in the name of Veronica Navera;
3) Subject duplicate copy of Transfer Certificate of Title No. T-26403 covers a

parcel of land located at Paringao, Bauang, La Union with an ares of Six hundred
twelve (612) square meters sold by Veronica Navera to Rufina N. Pinzon pursuant to
a Deed of Absolute Sale of a Registered Land acknowledged before Notary Public,
Donato B. Caloza and docketed in his Notarial Registry as “Doc. No. 84; Page No. 17;
Book No. CII; and Series of 2018;
4) The same owner’s duplicate copy of Transfer Certificate of Title was

entrusted to me for safekeeping by Rufina N. Pinzon after the sale was
consummated between Veronica Navera and Rufina N. Pinzon, and with the view of
having it transferred to her name once she decides to process the transfer and for
which she asked me to do;
5) Sometime in November, 2018, Rufina N. Pinzon decided to process the

transfer of title in her name and asked me to start the process and prepare all that
are necessary to effect the transfer;
6) I started preparing all the necessary documentations for the purpose only to
find out that I have misplaced the owner’s duplicate copy of Transfer Certificate of
Title No. T-26403 entrusted to me previously by Rufina N. Pinzon;
7) I extensively searched the same in my house and in the cabinet drawer

where I placed all my important document but failed to find the same;
8) I now could just surmise that the same document could have been
inadvertently misplaced during all the time (until now) I am very busy taking care of
my old and ailing mother, who is in and out of the hospital since last year;
9) In the process of my very hectic and often times tiresome activities of taking
care of my mother, I have failed to take care of my personal files and left them
disorganized in the process;

10) In view of my inability to find the same despite all the efforts I did to locate
it inside the premises of my house, I now consider it lost forever;
11) I reported the matter sometime in January 2019 to Rufina N. Pinzon who
understood the situation and asked me to execute instead this Affidavit to support
her application to have the parcel of land covered by the subject TCT No. T-26403
she bought from Veronica Navera transferred in her name;
12) I certify and confirm that the parcel of land covered by Transfer Certificate of
Title No. T-26403 of the Registry of Deeds of La Union was never mortgaged, sold,
conveyed, disposed of, or encumbered to any other person or entity. Moreover, I
certify and confirm further that the said document was neither confiscated, nor
relinquished in favor of any government entity, agency or authority; and
13) I attest to the truth of the foregoing statements to the best of my own
personal knowledge and based on authentic documents or records. I am executing
this Affidavit of Confirmation of Loss to support Rufina N. Pinzon’s application for a
new owner’s duplicate copy of Transfer Certificate of Title No. T-26403, as well as
for all lawful intents and purposes it may serve.
