This is a Self-Learning Kit intended for a 1 week lesson which is designed to assist
you understand easily the lesson without the presence of your teacher. It is
composed of short discussion, simplified examples and follow up activities for you to
understand deeper the lesson.
As always, read all the instructions and necessary information so that you can
follow the given activities.
After completing this SLK, it is then expected that you can answer the
assessment correctly and able to discern and apply the given objectives below.

2. Enumerate the procedures in requesting carpentry tools
and materials
3. Explain the parts of bill of materials
4. Fill up the requisition form and the bill of materials

Requisition is a written order or formal demand by a user of tools,
materials, and services to the department or person in charge. A form or
document used in making requests is called a requisition form. A material
requisition form list the items available in the inventory that may be used
during operation or production. This form is used in picking up items from the
stock and relieving the inventory records in the number of items picked.
Different people are oftentimes involved in the requisition process.
1. Requisitor-is the person in charge of the quality and quantity of the of the
materials and tools being requested.
2. Purchaser-is the negotiator that is responsible in keeping an operation or
production run smoothly by making sure that all the materials and tools needed
are available
3. Financial Controller-is responsible in checking the availability of budget to make
actions on submitted requests.
4. General Manager-is in charge of checking the request submitted

The following are the procedures on how to accomplish the form to


acquire materials.
1. State the name of the project and the date when you requested for materials.
2. Fill in the classification of the project and its purpose.
3. Write the number of required materials.
4. List the unit of materials to be requisitioned.
5. Enumerate the materials with their description.
6. List the cost per unit and the total cost.
7. Write your name as a requisitioner.
8. Secure the approval of authorities concerned.

Bill of Materials is a list of all things needed in a project together with their
description, sizes, and amount or price. All information should be in the bill of
materials.

Parts of a Bill of Materials

1. The quantity tells the number of materials to be purchased.


2. The unit gives the measure of materials to be purchased
3. The description states the detailed name of materials.
4. The unit cost gives the price of each materials.
5. The amount is the cost of all materials.

Hand Quantity Unit Description

A form or document used in making requests is called


requisition form. Different people involved in the requisition
process like the requisitor, purchaser, financial controller, and a
manager. A bill of materials is a hierarchical list of components
needed to build or manufacture a product and to create an
assembly. Before a bill of materials can be made, an inventory
of parts and a list of suppliers are needed.
