Professional Experience:
● Ensure individualized employment plan for enrolled participants is complete and coordinate with client and staff
as needed
● Works in a team environment with other public and private service providers, coordinates and manages clients'
participation in a variety of services
● Serves as policy expert in one or more program areas, providing guidance, direction, and information to other
staff
● Participate in monthly transition team meetings to delivering comprehensive update of each client from all
