    AFFIDAVIT OF UNDERTAKING

    I, ________________, in my capacity as the authorized representative of RNA


    Marine Services with office address at Unit C Lot 70 Block 20 Phase II, Brgy. Longos,
    Malabon City, do hereby and states under oath that:

    1. _________________ hereby applies for issuance permit of Permit to


    Operate (PTO) for SHIPPING AGENT SERVICES, SHIP CHANDLING
    SERVICES & HAULING/ TRUCKING SERVICES at the Base Port, Sub-Ports and
    Private Port of Batangas;

    2. RNA Marine Services shall submit a certified thru copy of its PPA
    issued Accreditation Certificate to PPA PMOB within 30 days and the other
    following lacking documents within 10 days from the date of the issuance of this
    application, viz:

    1. PPA Certificate of Accreditation or Proof of Submission of


    Application for PPA Accreditation & Original Copy of Duly Notarized Deed
    of Undertaking
    2. Batangas City Special Permit ( except for operations outside
    Batangas City
    3. Annual Renewal of BIR Registration (Form 0605 and Proof of
    Payment)
    4. Copy of most recent Audited Financial Statement

    3. RNA Marine Services agrees that the Permit to Operate issued by PPA
    shall automatically revoke for failure to submit the documents above.
