     Location: BF at twelve grids: 1E, 1B, 2A, 2B, 2D, 2E, 3D, 4A, 4B, 11D, 11E and 12E
     Issue: Installing rebar and concrete casting for 12 pile caps as mention in Location was done without
    inspection; on informing. Cast in early morning (1:25 to 4:30) on Jan 17, 2019, contractor finished rebar
    installation in the midnight and instantaneously concreted.
    Status: MAM disapproved for the 12 pile caps concrete casting.
     Date: 17.January.2019
