    “ESTABLISHING FOR THE BARANGAY CONTACT TRACING TEAM (CTT)”

    WHEREAREAS, as provided for under DILG Memo Circular No.2020-073 dated April 13,
    2020 each barangay are directed to organize its CONTACT TRACING TEAM (CTT);

    NOW THEREFORE, I ARTEMIO R. BUAN, Punong Barangay of Barangay Poblacion 1,


    Moncada, Tarlac by virtue of the powers vested in me by law hereby order the establishment
    of the Contact Tracing Team (CTT)

    SECTION 1. Establishment and Composition of the CTT.

    SECTION II. Primary Duties.

    1. To conduct contact tracing for all person with:

    a. Known exposure to a COVID-19 patient to ensure that they are strictly observing
    the home quarantine measures.
    b. Monitor the status of Persons Under Investigation (PUI), Persons Under
    Monitoring (PUM), family members with known contact to a confirmed case to
    mitigate possibility of contagion and;
    c. Report daily update to the COVID-19 Municipal and Provincial Task Forces

    SECTION III. This ORDER shall take effect upon its approval.

    Done this 22nd day of July in this year of our Lord, Two Thousand and Twenty at Barangay
    Poblacion 1, Moncada, Tarlac.
