    AFFIDAVIT
    (Support and Consent/Parental Travel Permit)

    I/We, RYAN M. LIM and MARICAR T. LIM, both of legal age, married,
    Filipinos and residents of Tabango, Leyte, Philippines, after having been duly sworn
    to in accordance with law do hereby depose and state:

    1. That I we are the biological father and mother and/or person exercising
    parental authority over minor ZIAN BLAKE LIM, a minor of five (5) years
    old;

    2. That the child, ZIAN BLAKE LIM, will travel to SINGAPORE and MALAYSIA
    for the following reason/s: to visit the aforementioned countries as a tourist;

    3. That the mentioned child/children will be leaving on June 6, 2018 and will
    stay in the said country for a period of SIX (6) days and will be taken cared
    of by Sps. ERLINDA LIM and RICARDO LIM;

    4. That we cannot personally travel with our child due to the following reasons:
    I, MARICAR T. LIM, will be working at Banco de Oro, Ormoc City, and I, RYAN
    M. LIM, will be manning our business;

    5. That I we are giving our full consent for the minor child to travel abroad to be
    accompanied by Sps. ERLINDA LIM and RICARDO LIM;

    6. That we personally guarantee the financial support of the child in the course
    of his travel and stay outside the Philippines and further guarantee that he
    will not in any manner become public ward/s of any authority of any country
    he may happen to pass en route to his destination;

    7. That this affidavit was executed for the purpose of attesting to the truth of
    the foregoing facts and for whatever legal purpose it may serve; and

    8. Affiant further sayeth naught.

    IN WITNESS HEREOF, I have hereunto set my hand this 22 nd day of May, 2018
    in Villaba, Leyte, Philippines.
