Generic Name: Celecoxib

Brand Name: Celebrex


Drug Classification: Analgesic (nonopioid); NSAID; Specific COX-2 enzyme inhibitor; Antirheumatics
Mechanism of Action: Analgesic and anti-inflammatory activities relate to inhibition of the COX-2 enzyme,
which is activated in inflammation to cause the signs and symptoms associated with inflammation; does not
affect the COX-1 enzyme, which protects the lining of the GI tract and has blood clotting and renal functions
Indications:

- Acute and long-term treatment of signs and symptoms of rheumatoid arthritis and osteoarthritis
- Reduction of the number of colorectal polyps in familial adenomatous polyposis (FAP)
- Management of acute pain
- Treatment of primary dysmenorrhea
- Relief of signs and symptoms of anklylosing spondylitis
- Relief of signs and symptoms of juvenile rheumatoid arthritis

Contraindications and cautions:

- Contraindicated with allergies to sulfonamides, celecoxib, NSAIDs, or aspirin; significant renal


impairment; perioperative pain post CABG surgery; pregnancy (third trimester); lactation
- Use cautiously with impaired hearing, hepatic and CV conditions

Side effects:

- Dizziness, drowsiness

Adverse effects:

- CNS: Headache, dizziness, somnolence, insomnia, fatigue, tiredness, tinnitus, ophthalmologic effects
- CV: MI, CVA
- Dermatologic: Rash, pruritus, sweating, dry mucous membranes, stomatitis
- GI: Nausea, abdominal pain, dyspepsia, flatulence, GI bleed
- Hematologic: Neutropenia, eosinophilia, leukopenia, pancytopenia, thrombocytopenia, agranulocytes,
granulocytopenia, aplastic anemia, decreased Hgb or Hct, bone marrow depression, menorrhagia
- Other: Peripheral edema, anaphylactoid reactions to anaphylactic shock

Nursing Responsibilities:

Before:

Assessment

- History:  Renal impairment, impaired hearing, allergies, hepatic and CV conditions, lactation, pregnancy

- Physical: Skin color and lesions; orientation, reflexes, ophthalmologic and audiometric evaluation,
peripheral sensation; P, edema; R, adventitious sounds; liver evaluation; CBC, LFTs, renal function
tests; serum electrolytes
During:

- Instruct patient to take drug with food or milk- Teach patient to avoid aspirin and other NSAIDs (such as
ibuprofen and naproxen) during therapy.
- Instruct patient to take celecoxib exactly as directed. Do not take more than prescribed dose.
- Instruct patient in correct technique for monitoring BP and to notify health care professional if
significant changes occur.

After:

- Advise patient to notify health care professional promptly if signs or symptoms of GI toxicity
(abdominal pain, black stools), cardiovascular effects (chest pain, shortness of breath, weakness, slurring
of speech), skin rash, unexplained weight gain, or edema occurs. Patients should discontinue celecoxib
and notify health care professional if signs and symptoms of hepatotoxicity (nausea, fatigue, lethargy,
pruritus, jaundice, upper right quadrant tenderness, flu-like symptoms) occur.
