INTRODUCTION

1. Student’s Log Book consists of Attendance Record and Clinic Diary.

2. This Log Book should be carried at all times during the rotation.

3. Students are expected to attend Dermatology clinics and wards from


Monday to Thursday to:

I. Observe methods of consultation, communication and counseling


skills practiced by the doctors.
II. To obtain a comprehensive history from the patient.
III. To perform a rational physical examination and identify the skin
lesion.
IV. To record in writing and present orally the results of complete history
and physical examination in a systemic and coherent manner

4. Students will be assessed via a viva voce based on the logbook on the last
day of the posting which will be a Friday of that week

5. Students are required to submit them at the end of the rotation for
evaluation.

6. Failing to comply with the above requirements will result in an unsatisfactory


evaluation.

7. The onus of ensuring the Log Book has been properly certified is on the
student.

8. Logbooks should be kept safely, and in good care

9. Please make a photo stat copy of the log book, should it go missing, as marks
will be deducted.

ATTENDANCE RECORD
Please ask your lecturer to sign in the allocated box at the end of each
session throughout your posting to confirm your attendance. 80% of
attendance is a pre-requisite to sit for the end-of-posting examinations.
