server and the Cedara I-Reach client application.

Web Server Operating Systems

You can install the Cedara I-Reach Web server on the Microsoft Windows
Server® 2008 R2 Standard (64-bit) English (US) operating system.

Caution: If new server hardware arrives with a pre-loaded operating

system, you must reload an appropriate operating system.

4
Chapter 1: Introduction

Client Workstation Operating Systems

You can install the Cedara I-Reach client application on the following English (US)
operating systems:

• Microsoft® Windows® 7 Premium, Professional and Ultimate (32-bit and 64-bit)

• Microsoft Windows Vista® (32-bit and 64-bit)

• Microsoft Windows XP Professional (32-bit and 64-bit)

Note: If you are using a 64-bit operating system, you must use the 32-bit
version of Microsoft Internet Explorer when running the Cedara I-Reach
client application.

Configuring the Operating Systems

For all operating systems, follow these guidelines:

• Use NTFS hard drive formatting.

• Use TCP/IP for the networking protocol. Refer to the Microsoft

Corporation’s documentation for instructions on how to install TCP/IP.

• The TCP/IP protocol must be correctly bound to a NIC adapter. The Web
server requires a unique IP address. If your network environment is
subnetted, then you must specify a default gateway IP address.

• Ensure that the network card or dial-up adapter on the Cedara I-Reach
client workstation can communicate with the Cedara I-Reach Web server.

• If your network does not use the Domain Name System (DNS), update the
host file on each client workstation with the IP address and host name of
the Cedara I-Reach Web server and the Cedara I-Store archive. When you
update the host file, you can access the Cedara I-Reach Web server using

5
Cedara I-Reach 4.5 Installation Guide

its host name (e.g., http://WebServerHostName/vrweb) instead of the IP

address (e.g., http://10.10.10.10/vrweb). You can locate the host file on
each client workstation at windir\system32\drives\etc where windir is the
location where the operating system is installed.

• Disable the Windows firewall on each Cedara I-Reach Web server and
Cedara I-Reach client workstation.

• If the system memory (RAM) is 3GB or more, use the /3G switch in the
boot.ini file for each Cedara I-Reach Web server and Cedara I-Reach client
workstation. Refer to Microsoft documentation on how to configure the OS.

Caution: For the Windows Server 2008 operating systems, do not install
the SharePoint option. If this option was previously installed on
your system, remove it. Your application will not function
properly if this option is installed on your system.

Precautions

The precautions described in this section will help ensure that you do not lose any
data from the workstation.

Pixel Spacing Parameters on Scanners

Ensure that your scanner’s Pixel Spacing and Imager Pixel Spacing parameters
are configured correctly, following the Digital Imaging and Communications in
Medicine (DICOM) standards.

WARNING: If the values are incorrect, the Cedara I-Reach application uses
the wrong pixel spacing to display images and print true-size
images.

6
Chapter 1: Introduction

Editing Files
Some of the procedures in this guide instruct you to edit files. We highly
recommend that you use the MS-DOS editor or Notepad to edit files. The use of
other text editors such as WordPad can cause unexpected errors by the way it
formats the file. If a file has already been edited using some other text editor, then
open it using the MS-DOS editor and save it to replace any incorrectly formatted
characters.

Caution: Do not use WordPad to edit text files.

Opening the Registry Editor

We strongly recommend you back up the registry before editing it so you can
recover from any accidents that might occur while you are working. When you
enter a value in the registry and the value is terminated by a backslash (\), you
must enter an additional backslash at the end of the string (e.g., to enter
D:\cedara\ you must type D:\cedara\\).

Caution: We strongly recommend you backup the registry before you edit
the registry.

The following procedures describe how to open the Registry Editor on the
Windows XP and Windows 7 operating systems.

> To open the Registry Editor on Windows XP

1. In the taskbar, open the Start menu and select Run.

2. In the Open field, type regedit and click OK.

> To open the Registry Editor on Windows 7

1. In the taskbar, open the Start menu.

2. In the Search Programs and Files field, type regedit and press ENTER.

7
Cedara I-Reach 4.5 Installation Guide

Date and Number Format

The application uses the date format and number format specified by the archive
server’s regional settings. You must configure the Cedara I-Reach server to
recognize four-digit years. You should also check your regional settings to ensure
that the archive server uses the most common date and number format used in
your region to avoid confusion.

> To check the regional settings

1. In the Taskbar, click Start, Settings, Control Panel.

2. In the Control Panel, double-click Regional and Language Options.

3. Ensure the correct language and number format are selected.

4. In the Regional and Language Options dialog box, click OK.

5. In the Control Panel, double-click Date and Time.

6. Ensure that the correct date, time and region are selected.

7. In the Date and Time Properties dialog box, click OK.

8. If you made any changes, restart the archive server.

Preventing Computer Viruses

Computer viruses can prevent the application from working properly.

Caution: We recommend that you install anti-virus software on all

workstations.

When performing a scan, do the following:

• Set the Heuristic Scanning Setting to the minimum level.

• Do not scan the \inetpub\wwwroot\ folder. This folder is the Cedara I-Reach
application directory.

• Do not scan the C:\Program Files\Microsoft SQL Server\MSSQL folder.

• Do not scan the C:\Temp folder.

• Do not scan any of the following extensions: .bmp, .db, .dcm, .gif, .inf, .jpeg,
.jpg, .mdb, .log.

About this Guide

This section describes various conventions and visual cues that are used
throughout this guide.

Drive Letter Conventions

When referring to drive letters, we make the assumptions listed below. If your
drive letters are different, then use yours instead.

• Hard Drive = C:

• Floppy Drive = A:

• CD Drive = D:

The examples provided throughout this guide assume that the application is
installed under the default path (e.g., for the Cedara I-Reach Web server
application, the default path is “C:\inetpub\wwwroot\Vrweb”).

Document Conventions
Throughout this guide, the following conventions and visual cues are used:

• Words shown in large, boldface text, such as Exit, indicate application

functions that you can click with the mouse (e.g., buttons, tools, items in
dropdown lists).

• Words shown in Times New Roman, boldface text, such as Administrator,

indicate characters that you must type exactly as they appear (i.e., if you are
instructed to log on as Administrator, you should type the characters exactly
as they are printed).

• Words shown in uppercase, such as ENTER, indicate keys on the keyboard

that you press. If several keys appear together separated by plus signs (e.g.,
CTRL+ALT+DEL), it means that you should press all three keys simultaneously.

• Words shown in italics are used for emphasis.

• Words shown in courier font indicate system responses.

Note: Note messages are used to indicate information which may be helpful or
of special interest to you.

Chapter 2: Installing the Cedara I-Reach Web

Server

This chapter describes how to install, license, and uninstall the Cedara I-Reach
Web server application.

Note: Due to potential impact on server performance, we do not recommend

that you install the Cedara I-Reach client application and the
wCedara I-Reach Web server on the same machine.

When you install the Cedara I-Reach Web server application, the installation
automatically creates the following:

• A default Super Administrator account whose user ID and password are

superadmin.
• A default group for all Super Administrator accounts called SuperAdminGroup.

• A default role for all Super Administrator accounts called SuperRole.

• A default group called GlobalSiteGroup that the Cedara I-Reach Web server
application uses when creating new roles. If you have created additional
groups, you can select those groups when you create new roles.

For details on administrating your Cedara I-Reach environment (e.g., creating

users, roles and groups), refer to the Cedara I-Reach Administrator’s Guide.

Note: If you are not installing the Cedara I-Reach Web server on the same
machine as the Cedara I-Store archive, after you complete all the steps in
this chapter, make sure you perform the steps in “Configuring the

Chapter 3: Configuring the Cedara I-Reach

Web Server

This chapter describes how to configure the Cedara I-Reach Web server. These
configurations include how to add a Cedara I-Store archive as a data source for
this Web server. The majority of the Cedara I-Reach Web server’s configurations
are performed by modifying the Cedara I-Reach Application and Server settings
pages.

When configuring the connection to the Cedara I-Store archive, note that the
Cedara I-Reach Web server can only connect to a single Cedara I-Store archive
which is either an online image cache or a near-line image archive.

Caution: A Cedara I-Reach Web server can only be associated with one
Cedara I-Store archive. If you need to add additional
Cedara I-Reach Web servers in the Server Settings page, those
Web servers must connect to their own Cedara I-Store archive.

In a multiple Web server configuration, the server settings for each

Cedara I-Reach Web server must be the same. For details and examples of
different Cedara I-Store configurations, refer to the Cedara I-Store Installation and
Configuration Guide.

Note: When using a multi-site configuration, make sure that all your
Cedara I-Reach servers are using the same transfer protocol type (HTTP
or HTTPS).

Note: If you want to run the Cedara I-Reach client application on the
Cedara I-Reach Web server, you must install Microsoft Internet Explorer
8.0.

Note: You must configure the Cedara I-Reach Web server to recognize
four-digit years (see “Date and Number Format” on page 8).

Configuring the Central User Preferences

This section describes how to configure the Central User Management Server on
the Cedara I-Reach Application Settings page. When you enable the central user
preferences feature, all user preferences reside at a central data center and
allows users who move from site to site to access their user preferences from any
remote server.

Note: All Cedara I-Reach Web servers must be configured to use the same
protocol, HTTP or HTTPS. If your servers are not using the same
protocol, the applications cannot communicate with each other.

To configure your servers to use this feature, an administrator must configure

each remote Web server to point to the central server’s user preferences
database. When this feature is configured, a user account only needs to be
created on the central server. Once the account is created on the central server,
that account can be used to log on to any of the remote servers (if the remote
server is pointing to the central server’s user preferences database).

The following procedure describes how to configure the remote sites to point to
the central server’s user preferences database.

Configuring the Cedara I-Reach Web Server

when the Cedara I-Store Archive is on a
Different Server

For dual-server (i.e., the Cedara I-Reach and Cedara I-Store applications are
installed on two separate servers) and multiple Web server configurations, do the
following on the Cedara I-Reach Web server before you add the Cedara I-Store
archive as a data source:

• Configure a virtual directory that defines where to find the image pool folder on
the Cedara I-Store archive (see “Configuring a Virtual Directory” on page
50).

• Configure the ODBC connection to the Cedara I-Store archive (see “Adding
the Connection to the Cedara I-Store Archive” on page 54).

49
Cedara I-Reach 4.5 Installation Guide

Configuring a Virtual Directory

For dual-server and multiple Web server configurations only, you must set up a
virtual directory on the Web server that indicates to the Cedara I-Reach Web
server application where to find images on the Cedara I-Store archive. For single-
server configurations, this is done automatically during installation; therefore, you
only need to verify the settings.

Note: If you have configured the Cedara I-Store archive as a Multi-site Image
Archive, you do not need to set up a virtual directory. If you are using this
configuration, then perform the necessary configurations on the central
server and each of the remote servers (see Chapter 4).

The first virtual directory is normally called “imagePool1”, where “1” is the image
pool index. If the Cedara I-Store archive has more than one image pool, then you
must create additional virtual directories with the appropriate image pool index
(e.g., imagePool2, imagePool3).

> To set up a virtual directory

1. In the Taskbar, click Start, All Programs, Administrative Tools, Internet
Information Services (IIS) Manager.

2. In the Connections panel, use the expander buttons to navigate to the

image pool index. If the Cedara I-Store archive has more than one image
pool, then you must create additional virtual directories with the
appropriate image pool index (e.g., imagePool2, imagePool3).

5. In the Physical Path field, type the full physical path to the image pool on the
Cedara I-Store archive (i.e., \\HostName\imagePool where HostName is the
host name of the Cedara I-Store archive).

Note: If you are using dual servers for your Cedara I-Store archive, make sure
you type the host name of the compression server. For details, refer to the
Cedara I-Store Installation Guide.

Note: The imagePool folder must be shared and Web shared on the
Cedara I-Store archive.

3. In the Memory Setting group box, type the memory value (in MB) in the
following fields, as required:

• Swap Size — The swap file temporarily stores image data when memory
pools are exhausted. If you anticipate the loading of a large numbers of
studies, the default value might not be sufficient and you should adjust it.
Use judgement and experience to adjust the Swap Size setting to reflect
the particular system and usage environment.

• Max. Memory — The maximum amount of physical memory the processing

server uses to create memory pools. The InstallShield Wizard determines
and configures the Max. Memory setting automatically. However, if you
wish to change the Max Memory setting or for details on the maximum
allowable value on a 32-bit and 64-bit system, refer to the guidelines
provided in the Cedara I-Reach Service Manual.

• Memory Pool — The processing server obtains memory from the system in
increments called memory pools that are used to store images being
displayed on the screen. If the application needs to display images larger
than 100 MB, then you need to increase the Memory Pool setting.

4. If you changed the Memory Pool value in Step 3 above, open Windows
Explorer, go to the C:\VRS\temp\3203 folder, and delete any memory pool
files that start with M.P.

5. To save your changes, click Save. If you do not want to save your changes,

Configuring the Skin Line Feature

This section describes how to configure the Skin Line feature using the Service
Tool. The Skin Line feature is an automated tool that determines the skin line
boundary of an image and uses this information to position the image in the
viewport. The Skin Line feature facilitates the following tools: Optimum View,
Zoom Fit, Fit to Viewport, Invert, and Breast Alignment.

WARNING: When using the Skin Line feature with Fuji CR Mammography
data, the viewport may not display the entire anatomy. We
currently do not support the Skin Line feature for use with Fuji
CR Mammography data. If you are using Fuji CR
Mammography data, we highly recommend that you disable
the Skin Line feature for this manufacturer. Refer to the
procedures in this section for details on how to disable the
Skin Line feature for this specific manufacturer.

WARNING: Incorrect skin line detection behavior occurs with Fuji CR

Mammography data; however, it can potentially occur with data
from other manufacturers as well. This problem can occur
when the image background is noisy or when image quality is
poor due to artifacts, incorrect acquisition, hardware issues
and digitized film, for example.

Note: These settings are saved on the local workstation and only apply to that
Cedara I-Reach client workstation.

feature, you will also disable the following tools: Optimum View, Zoom Fit,
Fit to Viewport (i.e., using skin line detection), Invert, and Breast
Alignment.

114
Chapter 6: Configuring the Cedara I-Reach Client

4. Do any of the following:

• The Quality dropdown list defines the smoothness of the Skin Line. The
default value is High. If you want to change this value, in the Quality
dropdown list, select the desired value.

• The dilation value specifies the margin of space between the skin line and
the actual breast tissue to prevent operations such as fit to viewport and
invert from inadvertently cutting off tissue. The dilation value is specified as
a fraction of the diagonal of the image. The default value is 0.015. If you
want to change the dilation value, in the Dilation field type the desired
value. This value must be between 0.00 and 0.10.

• When the Use Skin Line when Fit to Viewport checkbox is selected, MG
images are zoomed to fit the view ports based on the skin line bounding
box. When the checkbox is clear, MG images are zoomed to fit the view
ports based on the entire image (including the background). Select or clear
the Use Skin Line when Fit to Viewport checkbox to produce the desired
behavior.

5. To save your changes, click Save. If you do not want to save your changes,
click Reset and all settings return to their last saved state.

6. When the system informs you need to restart the application, click Yes.

Chapter 6: Configuring the Cedara I-Reach Client

Modifying the Pixel to Millimeters Conversion

Settings for Specific Manufacturers and
Modalities

The Cedara I-Reach application allows you to specify how it converts pixels to
millimeters (mm) when creating measurements. The conversion is done based on
the values of three tags in the DICOM image header:

• Pixel Spacing (0028,0030);

• Imager Pixel Spacing (0018,1164); and

• Estimated Radiographic Magnification Factor (ERMF) (0018, 1114).

You can customize these conversion settings for specific manufacturers and
modalities. For example, you can configure MG images from Manufacturer A to
use imager pixel spacing and CR images from Manufacturer B to use pixel
spacing. Table 6-5 describes the three registry keys responsible for defining the
conversion method the application uses for specific manufacturers and
modalities.

Table 6-5: Pixel Conversion Registry Keys

To select a conversion method, the Cedara I-Reach application searches the

registry for the three keys listed in Table 6-5. In the registry, you define these
three keys for a specific manufacturer, modality, model name or software version
combination. The location of these three keys in the registry hierarchy defines
which images are affected by their values. If an image’s DICOM values match a
combination defined in the registry, the application uses the conversion method
defined by the three keys in that location. If the application cannot find registry
values that match the image’s values, it uses the default conversion method
defined in the root location (Pipe).

6. In the OEM Integration group box, modify the following OEM application
settings as required:

• In the OEM Invocation Password (OEM Key) field, type the password that
the user group uses to access the OEM's user interface.

• In the OEM User Group Name dropdown list, select a user group name to
which new OEM users will belong.

• Select the "Ignore Patient Information" checkbox if you want the application
to ignore patient information while loading studies into the application. Note
that in OEM mode, the compare feature is not supported. Therefore, if
studies from the same patient have even a minor difference, the studies will
be treated as different patients.

Caution: If you select the Ignore Patient Information checkbox, all studies
will be sent to the application regardless of the patient
information. You must verify that the study you are viewing is
associated with the correct patient.

7. After you make the necessary changes, click Update, then close the browser
window.

Customizing the Cedara I-Reach Client Application

You can customize how various items appear on the Cedara I-Reach client
application, such as, the user templates and registry settings.

Chapter 7: Configuring the OEM Settings

Configuring the Study Report Feature

The study report is displayed in a separate browser window, using a server-side

script that retrieves and generates the required HTML output. You can use an
ASP script, a Perl script or JavaScript to configure the information that is
displayed in the study report.

On the Cedara I-Reach Web server, the script is invoked in the file
C:\inetpub\wwwroot\Vrweb\wrklsthdr.asp, under the displayReport() function. The
Show Report section in the wrklsthdr.asp file is shown below.

141
Cedara I-Reach 4.5 Installation Guide

The script generates the following identical report for all studies.

To configure the study report, you can change the definition of the reportURL
variable to point to the location where the study report is retrieved.

For example, the Mitra Report Broker has a report.pl script, which accepts the
patient ID and accession number as keys to retrieve the study report. The
reportURL should then be changed as follows:
//Show report
var reportURL = "http://mitraBroker.someClinic.com/cgi-bin/report.pl?"
+
"&patient_id=" + patID+
"&accession_number=" + accNum;
