AT A MEETING OF THE PARENT TEACHERS’ ASSOCIATION OF FRANCISCO ORINGO SR.

ELEMENTARY SCHOOL, ORINGO SUBD., BRGY. CITY HEIGHTS, GENERAL SANTOS CITY HELD AT
FRANCISCO ORINGO SR. ELEMENTARY SCHOOL ON August 15, 2015, (First BOD Meeting),

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School, Brgy.City Heights,
General Santos City, resolved as follows:

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School supports the schools’
programs and projects and joins in any school activities in line with its mission, vision and goals
for School Year 2015 – 2016.

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School takes actions and
responds to the arising problems and the like of the school for School Year 2015 – 2016.

WHEREAS, it is necessary and desirable that General PTA of Francisco Oringo Sr. Elementary
School coordinate their efforts to effectively and efficiently allocate funds for the needs of the
school and the children.

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School initiates fund raising
activities to which proceeds goes to the school needs and projects.

NOW, THEREFORE, BE IT HEREBY PROCLAIMED by General PTA of Francisco Oringo Sr.
Elementary School officially takes its full responsibilities in ensuring that the school achieves its
goals towards the holistic development of the school children and the like.

FRANCISCO ORINGO SR. ELEMENTARY SCHOOL ON August 15, 2015, (First BOD Meeting),

RESOLUTION - ADOPTING THE DepEd Order No. 54 s. 2009 the REVISED GUIDELINES GOVERNING
PARENT TEACHERS ASSOCIATION (PTAs) AT THE SCHOOL LEVEL.

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School, Brgy. City Heights,
General Santos City, resolved as follows:

WHEREAS, Section VIII No. 2 Safekeeping of Funds states that all collections of contributions
or proceeds of fundraising activities shall be deposited in a reputable banking institution as
determined by the Board of Directors.

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School opens a bank account
for Francisco Oringo Sr. Elementary Schools’ safekeeping of funds.

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School determines the
signatories of the disbursement of funds from the bank are as follows; Mrs. Densie Jane
Valenzuela - PTA Treasurer, Bebelita D. Catalan – PTA President, and Mrs. Ma. Angelica E. Figueroa
– School Principal.

NOW, THEREFORE, BE IT HEREBY PROCLAIMED by General PTA of Francisco Oringo Sr.

Elementary School that the safekeeping of PTA contributions and funds be deposited in a
reputable bank.

use of the existing account of Francisco Oringo Sr. Elementary School/PTA bearing the account
no. 02509 for safekeeping of funds.

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School determines the
signatories of the disbursement of funds from the bank are as follows; Mrs. Densie Jane
Valenzuela - PTA Treasurer, Bebelita D. Catalan – PTA President, and Mrs. Ma. Angelica E. Figueroa
– School Principal, therefore change the previous signatories; Mrs. Miraflor Palacios and Mrs. Juliet
Tan with the new signatories under oath.

NOW, THEREFORE, BE IT HEREBY PROCLAIMED by General PTA of Francisco Oringo Sr.

Elementary School to continue the safekeeping of PTA contributions and funds to the existing
account of Francisco Oringo Sr. Elementary School/PTA.

ELEMENTARY SCHOOL, ORINGO SUBD., BRGY. CITY HEIGHTS, GENERAL SANTOS CITY HELD AT
FRANCISCO ORINGO SR. ELEMENTARY SCHOOL ON SEPTEMBER 30, 2015, (Second BOD Meeting),

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School, Brgy. City Heights,
General Santos City, resolved as follows:

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School approves the
installation of Water Facilities of the school through GSC Water District for five (5) buildings inside
the school area.

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School recognizes that the
allocated funds/budget for the installation of five (5) water meter comes from the winning prize
of the school from the “MODELO ANG PAARALAN KO” Contest amounting to eighteen thousand
eight hundred seventy-two pesos and thirty –five centavos only (₱18, 872.35).

WHEREAS, the General PTA of Francisco Oringo Sr. Elementary School allocates funds/budget
for the labor and materials needed for 25 classrooms and 1 caretaker’s house coming from the
PTA Project last SY. 2014 -2015 and the proceeds of the PTA initiated FUN RUN for School Year
2015-2016 amounting to eighteen thousand two hundred pesos only (18,200.00).

NOW, THEREFORE, BE IT HEREBY PROCLAIMED by General PTA of Francisco Oringo Sr.

Elementary School to push through the installation of Water Facilities of the school through GSC
Water District for five (5) buildings provided with the funds allocated.
