On August 27, 2004 at around 2:30pm, three workers at the Crushing Plant's scalping screen were injured when cleaning build-up material from inside the discharge chute. While two employees were using a ladder and pick inside the partially blocked chute, the built-up material dislodged and trapped them. The third employee standing on the locked out conveyor belt fractured his lower right leg. An investigation found the immediate causes were allowing entry into the confined space where material could slip, failure to follow safety procedures, poor illumination, unsafe design, and lack of risk assessments. Preventive actions will be pending the results of a full investigation.

DESCRIPTION OF INCIDENT (WHO, WHAT, HOW)
Three workers entered the partially choked discharge chute of a scalping screen and positioned a ladder via an access point between the conveyor and the bottom of the chute. One employee held the ladder in position and the second person climbed on it with a pick to remove build-up material on the side of the chute. The third employee was positioned on the locked out conveyor belt surface, holding a torch that provided the illumination inside the chute. Whilst the cleaning activity was in progress, starting from the top, the build-up became dislodged and trapped the employee holding the ladder and the employee holding the torch. Injuries The employee holding the torch on the locked out conveyor surface, sustained a fracture to his right lower leg. Immediate Causes Allowing employees to enter the confined space with potential for material to slip. Failure to follow procedure. Poor illumination. Unsafe design. Lack of Risk assessments. PREVENTIVE ACTION Pending Results of a Full Investigation.
