ACCIDENT DURING LADDER CUTTING OPERATION


HIGH POTENTIAL ACCIDENT

GENERALITIES

Accident Location: Plant Project, Qatar


Date of the accident: 29th of July, 2012
Type of activity: Installation of additional ladders on top grating platform

The supervisor and four workers had


to install three additional ladders at
the top grating platform, in order to
improve the access to the valves area.
After the last ladder was installed,
they observed that the lower part of
the ladder channel is unfit for its
allocated position, being few
centimeters longer. Therefore they
decided to cut this part with a 7” disc
grinder (approx. 5 kg weight).
The injured person, iron foreman, was
assigned to perform the cutting
operation.
The working area was congested and
in order to carry out the operation the
iron foreman positioned over the open
grating with his left leg very close to
the power tool (see picture no. 1).
During the cutting process, the disc
stuck and got damaged (see picture
no. 2) and then grinder became
unstable. The injured person was grating
unable to control it so the tool moved opening
towards his left thigh, causing a deep

POSSIBLE CONSEQUENCES:
• severe deep cut/amputation of the leg;
• fall through the open grating hole

CAUSES OF THE ACCIDENT

• Working environment: the work area was congested and the platform was not secured,
containing grating openings.
• Position for task: the foreman placed himself in an unsafe position, over a platform grating
opening, with left leg too close to the grinder.
• Activity’s planning: hot work permit was issued for installation of additional ladder. No JSA
or TBT was performed for working at height, in congested area.
• Power tool unfit for purpose: the grinder weighted approx. 5 kg and it was not inspected
prior to be used by the injured person.
• Exposure to extreme conditions: the injured person was performing his activity over working
hours; the heat index was between 50-53 (double orange flag).

CORRECTIVE ACTIONS

• Ensure that the activity is performed in a proper/safe working environment. Assessment of


the working area must to be carried out prior to start the activity.
• Ensure the inspection of power tools prior to be used. They have to be fit for the purpose and
marked with the proper color code.
• Limit the works performed at height during periods with hot weather conditions.
• Ensure the closely supervision of the activities performed at height and stop the activity
immediately if any worker does not comply with the safety practices.
• Ensure the time on/off rotation of all the workers is respected.
