Teachers Guidelines
Prepare two important individual documents namely Yearly Scheme of work (RPT) and

DSKP.
Check thoroughly all the skills (learning standard) provided in the ‘Rekod Semakan
Pentaksiran Murid' template.

* Important consideration need to be made :- ensure all skills which are stated in the
template are alligned with the ones available in DSKP and RPT.
Only assess the skills which are available in the RPT document using selected teacher's
instrument or strategy.

*Conveniently mark the skills in the template. This indicates the skills which teachers
intend to assess based on individual RPT.
Begin the formative assessment & record the grade (Band 1 - 6) according to tested skill

in the template.
Summative assessment will only take place as soon as teacher completes the required

skills for formative assessment.
The amount of ‘Rekod Semakan Pentaksiran Murid’ template is based on the frequency
of assessment which teacher conducts in the classroom according to topic.
