DEED OF EXTRAJUDICIAL SETTLEMENT OF ESTATE

OF XXXXXXX

KNOW ALL MEN BY THESE PRESENTS:

This Deed of Extrajudicial Settlement of the Estate of XXXXX is entered into


this21st
day of July 2016, in Makati City, made and executed by and among legal
and compulsory heirs of the deceased namely:

Juliana, of legal age, Filipino, widow, residing at, Dasmariñas Village,


Makati City;

Ceferino of legal age, Filipino, married, residing at Ayala Alabang


Village, Muntinlupa;

Sixto, of legal age, Filipino, married, residing at Ayala Alabang


Village, Muntinlupa;

Lynette, of legal age, Filipino, married, residing at residing at


Dasmariñas Village, Makati City;

Bernardo of legal age, Filipino, married, residing at Dasmariñas


Village, Makati City;

Joseph, of legal age, Filipino, married, residing atGatchalian


Subdivision, Las Pinas City ; and

Vince, minor, Filipino, residing at BF Homes, Paranaque City,


represented by Vicky, his legal representative, duly authorized for this
purpose.

WITNESSETH, that -

1. XXXX (the “Decedent”) died intestate on 24 January 2016 at Fort


Bonifacio, Taguig City (a copy of his death certificate is hereto attached as Annex
“A”), without Will or Testament, and without any outstanding debts in favor of any
person or entity.

2. The Decedent’s surviving legal and compulsory heirs are the herein
PARTIES/HEIRS, namely:

a) JULIANA– wife
b) CEFERINO– legitimate son
c) SIXTO - legitimate son
d) LYNETTE– legitimate daughter
e) BERNARDO– legitimate son
f) CEFERINO– illegitimate son
g) VINCE– illegitimate grandchild
representing Vincent illegitimate son of Ceferino, deceased.

A copy of the marriage certificate is attached hereto as Annex “B.”

Copies of the respective birth certificates of the children and grandchild are
attached hereto as annexes “C,” “D,” “E,” “F,” “G,” and “H.”

3. To the best of the PARTIES/HEIRS’ knowledge, at the time of his


death, the Decedent left the following properties:

I. Exclusive Real Properties (Land)

A parcel of land located in Tanauan City, Batangas, covered by


TCT No. 60902 of the Registry of Deeds of Tanauan City, the exact
description of which is indicated on the face of the title, a copy of which
is attached herein as Annex “I”. [BETTER TO TYPE IN THE DETAILS
OF THE PROPERTY AS STATED IN THE TCT. WE ANNEXED ONLY
FOR EXPEDIENCY.]

II. Conjugal Real Properties (Land and Building)

A parcel of land located in Santolan, Pasig City, covered by TCT


No. 292880 of the Registry of Deeds of Pasig City, the exact description
of which is indicated on the face of the title, a copy of which is attached
herein as Annex “O”.

A parcel of land located in Santolan, Pasig City, covered by TCT


No. PT-136074 of the Registry of Deeds of Pasig City, the exact
description of which is indicated on the face of the title, a copy of which
is attached herein as Annex “P”.

A residential house in Batangas East, Ayala Alabang Village,


Muntinlupa City, covered by Tax Declaration No. F-009-10008, issued
by the City Assessor of Muntinlupa, the exact description of which is
indicated on the face of the tax declaration, a copy of which is attached
herein as Annex “FF”.

III. Conjugal Personal Properties

4. The PARTIES/HEIRS have agreed that prior to the distribution of the


assets of the estate of the Decedent, all related taxes and expenses incurred in settling
the estate shall be advanced by the legitimate heirs and shall be borne by all the
PARTIES/HEIRS in accordance with their share in the estate.

5. The PARTIES/HEIRS, by these presents, voluntarily agree to settle the


estate of the Decedent and have agreed to divide and settle the aforementioned
estate between them in accordance with the rules of intestacy. Accordingly, pursuant
to Rule 74, Sec. 1 of the Revised Rules of Court of the Philippines, the herein
PARTIES/HEIRS freely and intelligently agree to settle the estate of the Decedent
and adjudicate among themselves the above-described properties in the following
manner:

CONVEY unto my co-heir NAME, his heirs and assigns the full ownership
and possession of the above-described properties.]

IN WITNESS WHEREOF, we hereunto set our hand on the 21st day of July,
2016 at Makati City, Philippines.

each page, and on the spaces provided on page 6.
