Sub: Physical verification and Test audit of accounts of Birth and Death Certificates
pertains to DHO (Health)/CZ for the year 2016
Introduction:-
The Commissioner, North DMC has directed to conduct a special Audit of all CSB
centers under jurisdiction of North DMC by Internal Audit Department for the period w.e.f.
01.01.2016 to 31.12.2016 to unearth the possibility of missing certificates (Birth/ Death) in
various Zones.
The audit on this account was conducted by Zonal Audit Party, City Zone comprising of
Ms. Swaran Kumari, AO, Sh. Prasad, AAO, Smt. Kiran Negi, HC, Sh. Subhash Chand Mishra
AA and Sh. Sandeep Kumar Vashist AA with working days w.e.f.01.02.17 to 15.03.17 (43
working days).
AIMS AND OBJECTIVES
The main aim of the CSB is to register the birth & death in MCD vicinity and issuing of
birth & Death certificates to the public.

Maintenance of Records:-
Since most of the chain of records were either not properly maintained by the department
concern or not produced to audit, the maintenance of records by the Deptt. Concerned cannot be

irregularities were issued.

Current Audit
In compliance of the orders of Worthy Commissioner a special Audit of Birth and

Death Certificates issued from Citizen Service Bureau-CZ was conducted w.e.f.
01.02.2017. The following record was requisitioned for audit:-
1. The stock and issue register of Blank certificates.

2. Whether number of certificates issued, utilized and returned were tallied at the
end of the day by Zonal Health Authorities or not?
3. Details of individual counter clerks/ DEO’s who performed duty during the
period in question.
4. Whether any login ID passwords were issued to individual DEOs on Zonal CSB
by IT Deptt for doing transaction related to Birth/Death certificate? Method of
transactions done by DEOs be explained.
5. Individual CSB counter wise details & date wise details of receipt, issue,
unutilized/ blank & cancelled forms.
6. Daily record of blank certificates held, issued & received back by Zonal sub-
registrar.
7. Criteria for issuing blank certificates to DEO/counter clerks be provided.

The Office of OIVS has provided the on-line data of Birth and Death Certificates for
the year 2016 on 08.03.2016 which starts from S. NO. 1392654 as on 1.1.2016 (issued to
Health Deptt.-CZ prior to January, 16) and ends with certificates no. 864767 as on
31.12.2016.
The internal Audit report has been prepared on the basis of information furnished
and records produced by the department. The paras are prepared on the basis of
reconciliation of the data of B&D Certificates issued to Zonal Health Department-CZ with
the online data of used certificates provided by the office of OIVS This party disclaims any
responsibility for any misinformation and/or non-information on the part of auditee.
PARA NO. 1:- CERTIFICATES NOT FOUND IN ONLINE DATA

As per the directions of the higher authorities the data of B&D Certificates, for the year
2016, issued from the office of OIVS to Zonal Health Department-CZ and its further distribution
to CSB operators with the online data of used certificates is reconciled. During the curse of audit

it has been noticed that although all the certificates which have been issued from the office of
OIVS are entered in zonal stock register-CZ and all are distributed to CSB operators in this year
but the usage of many certificates are not found in online data. The detail of the same is enclosed
as Annexure “A”.
Total Missing certificates in online data- 80825
In view of the above, the department was requested to appraise the whereabouts of above
certificates to audit. Being a serious issue concerned officers are requested to look into the matter
and ask his officials about the usage of above missing certificates under intimation to audit.
PARA NO. 2:- CERTIFICATE NUMBERS NOT PROVIDED OR

MENTIONED ZERO
During the course of audit while checking the online data of Birth and Death
Certificates pertain to CSB-CZ for the period 01-01-16 to 31-12-16 so provided by the OIVS on
08.03.2016, It has been noticed that 1479 (Approx.) certificates are issued without certificate
numbers or Zero have been placed/feeded in place of the actual printed certificate numbers
while issuing the certificates to general public. The details of some such cases are attached as
Annexure “B”.
Department is requested to explain and justify the reasons for above discrepancies.
PARA NO. 3:-VAGUE CERTIFICATE NUMBERS

During the course of audit while checking the online data of Birth and Death Certificates
pertain to CSB-CZ for the period 01-01-16 to 31-12-16 so provided by the OIVS on 08.03.2016,
It has been noticed that vague certificates 2158 (Approx.) in numbers or dates have been
shown/feeded in place of the actual printed certificate numbers while issuing the certificates to
general public. The detail of the some such cases are attached as Annexure “C”.
Department is requested to explain and justify the reasons for above discrepancies.
PARA NO. 4:- CERTIFICATES OF SAME SERIAL NUMBERS USED FOR

BIRTH AND DEATH
During the course of audit, series of certificates issued from the office of OIVS to Health
Department-CZ is checked. It is found that in the year 2016 no Series of certificates is found
repeated. But while checking the online data of B&D certificates it has been found that
certificates of the same serial number are issued for Birth as well as for Death to different people
with different registration number, G-8 numbers and on different dates. However summarized
data is as below :-

Audit is unable to understand how such type of discrepancy occurred because CSB
centers are programmed and fully computerized. So, the possibility of manual interference in
feeding the serial number of certificates at any stage cannot be ruled out by audit. Being a serious
matter department was asked to explain and justify the reasons for above discrepancies.
PARA NO. 5:- BLANK CERTIFICATES USED WITHOUT ITS ISSUANCE
The data of B&D Certificates 8442 (Approx.) for the year 2016, issued from the office
of OIVS to Zonal Health Department-CZ and its further distribution to CSB operators with the
online data of used certificates is reconciled. During the course of audit it has been noticed that
the following certificates are neither issued from the office of OIVS nor from zonal stock
register-CZ to CSB operators but the usage of the same are found in online data.
The details of some such cases are enclosed as Annexure “D”.

PARA NO. 7:- NON-PRODUCTION OF DISCARD/CANCELLED B&D

CERTIFICATES
During the course of audit, to reconcile the data of B&D certificates, Health department
was asked to provide the list of discard/cancelled certificates vide this office Letter No
IAD/ZAP/2017/01 dated 30.01.17 which was not provided by the OIVS. In the meanwhile, list
of said certificates was received from the department in which only 218 certificate nos. are
canceled during the period from 1.5.2016 to 31.12.2016 with remarks “NOT PRINT
CERTIFICATE OR PRINTER PROBLEM” are shown discarded in city Zone. The department
is requested to give a certificate that to ensure that no other discard certificates are lying with the
department in this regard except following lists, under intimation to audit.
A. On verification of the records, the below mentioned certificates were found in the list
provided by the department, but the same were not produced for physical verification.

PARA NO.08: CASTING MISTAKE

During the scrutiny of the Stock register (Blank Certificate) for the year 2015-16
maintained by the department, the following mistakes in computing balances/casting have been
noticed:-

The above said mistakes are of serious in nature and there is every possibility of misusing
of the blank certificate. Hence, the authority competent is directed to make periodical checking
of the stock register maintained by the department. The department is also directed to rectify the
mistakes/short coming at the earliest under intimation to audit.
PARA NO. 9:- IRREGULARITIES IN STOCK REGISTER OF BIRTH AND

DEATH CERTIFICATES
During the course of audit while checking the stock registers for Birth and Death
certificates for the period from 04.11.2015 to 31.03.2016 & 01.04.2016 to 28.01.2017 except
previous stock register so produced by the department, it has been observed that the said record
is not maintained properly and following discrepancies have been observed:
i. On scrutiny of the blank B&D certificate Issue Register of CSB/KH it has come
to the notice that on August 15th 2016 they have issued B&D
certificates to the General public, which was National Holiday . Similarly,
B&D certificate were issued on 25th December 2016 by CSB/CZ which
was also National Holiday

ii. Record pertaining to Indent/receipts of Birth & Death certificates from the HQ/
other zones is not maintained.
iii. While making the entries of Birth & Death certificate received from the HQ
series/packet no. is not recorded in Main stock book, only numbers are written.
iv. Day to Day balance of certificates is not recorded after its issuance.
v. Certificates are not issued to CSB in proper sequence.
vi. The certificates are issued in bulk quantity directly to data entry operators of CSB.
vii. Utilization record of used certificates is not maintained by the department.
viii. Individual CSB counter wise details & date wise details of receipt, issue,
unutilized/ blank & cancelled certificates is not maintained.
ix. Daily record of blank certificates held, issued & received back by Zonal
sub-registrar is not maintained.
x. No paging and paging certificate is made by the concerned official
xi. There is lots of cutting and over writing is made in the register.
xii. This register is not periodically checked by senior officers.
Due to such irregularities audit is unable to do the physical verification of these

certificates. Non-maintenance of proper record of each and every issue of certificate may become
cause of bogus Birth/Death certificate of any person at any stage, which is a serious lapse on part
of the working of the department. Such type of irregularity has also been pointed out in previous
audit reports, but no corrective measures have been taken so far. Further DHO is requested to
arrange to reconcile the record of certificates issued during the period of audit and furnish a
certificate in this regard.
PARA NO. 10:- REPETITION OF CERTIFICATES NUMBERS ON

DIFFERENT DATES
During the course of audit while checking the online data of B&D certificates pertain to
CSB-CZ, it is found that certificates of same serial number are issued to general public on
different dates in the year 2016. It is pertinent to mention here that as per the zonal record no
series of any certificate is repeated in the year 2016. Then repetition of same serial number on
different dates by CSB department is beyond comprehension of audit. Detail of the same is as
