INSTRUCTIONS: Rate him/her in each category by indicating the level of performance that most accurately describes
him/her performance in the community project. Use the performance level below to designate ratings:

5. ALWAYS/SUPERIOR – He/she consistently performs and manifests all assignments and work values at
a level significantly above the requirements and rarely achieved by others. Performance is characterized
by exceptional contribution and extraordinarily accomplishment.
4. OFTEN/ABOVE AVERAGE – His/her performance and work values continually exceed fundamental
requirements. He/she maintains a high level of achievement on a sustainable basis.
3. NORMALLY/AVERAGE – His/her performance and work values meet all requirements. He/she gradually
fulfills all key assignments satisfactorily.
2. OCCASIONALLY/BELOW AVERAGE – His/her performance and work values are at a less than fully
satisfactorily level. He/she may require more than normal assistance, guidance, and supervision.
Improvement is required.
1. RARELY/POOR – His/her performance and work values do not meet the minimum acceptable
requirements and demand close supervision and frequent review of work. Performance at this level is not
acceptable and demands immediate improvement.
