    Step 2 Obtain clearances from the following offices:

    Step 3 Pay account, if any, at the Finance Department. Present Official Receipt to the
    Students Account Section of Comptroller’s Office

    Step 4 Submit this accomplished clearance to the Principal’s Office, ESLS.

    NOTE: Transferring students will be given the F-138 as a transfer credential. The F-137 will be
    sent to the school concerned upon request.

    THIS APPLILCATION WILL BE INVALIDATED IF NOT FILED WITHIN ONE
