Q. no. 1 – where to file online application for enrollment as trainee surveyor, fresh
surveyor license, renewal, modification and for duplicate surveyor license?
Ans – on www.irdabap.org.in Bap portal.

01 - Eligibility to become surveyor/trainee surveyor

Q. no. 3 - how to determine the eligibility to become a surveyor/trainee surveyor?


Ans. – (Amended as per IRDAI ( Surveyor) Regulation 2015) please refer Schedule
I, Annexure 1 of IRDAI (Insurance Surveyors & loss Assessors) Regulation, 2015 on
qualification criteria for enrollment and licensing of surveyor and loss assessors.

02 – User id and password

Q. no. 4 - How to create/obtain user id and password?


Ans. – (a) For enrollment, applicants can create their own user id on portal
(b) For exiting surveyor license holder may request for user id and password on
[email protected]
(c) For fresh corporate surveyor license – the applicant can create their user id
and password
(d) For renewal of corporate surveyor license holder may request for user id and
password on [email protected]

Q. no. 5 - What happens if my user ID is deleted?


Ans. - If your account is deleted, then you have to re-register yourself in the portal if
you are a trainee. For already license holders- please contact IRDA.

Q. no. 6 - How much time it takes to get user id and password.


Ans – 72 working hrs

Q. no. 7 – Where to request to unlock the passwords for user id


Ans – (a) For application related to renewal, fresh, enrollment, modifications and
duplicates – Use the “FORGET PASSWORD/UNLOCK USER ID ” option provided on
BAP log on www.irdabap.org.in

03 - Procedure for enrollment as trainee surveyor on bap portal
www.irdabap.org.in.

Q. no. 9 – What are the Steps for enrollment? (Amended)


Ans.- 1. Click on the tab Surveyors then click on register for login for new applicant
2. Create user id and password
3. Update your profile
4. Select department(s)
5. Select name of your trainer from the list of surveyors available on the portal*
6. A 5 digit number will be generated after successful submission of training
request - no separate e-mail will be sent to trainer regarding same
7. Trainer will access the portal from his user ID and accept the training request.

8. Now trainee applicant will fill Enrolment Form No. 13. Unless trainer accepts the
training request (step 7 above), system will not allow applicant to proceed for filling
form number 13
9. Upload the following documents: - photo id proof, matching address proof as
feeded in application form, digital signature, digital photograph, Degree/diploma
certificate and online trainer’s acceptance certificate

After submission form number 13 successfully, URN – number will be generated


automatically

*In case name of your trainer is not available, your trainer is required to
contact IRDAI to add his name in the list of surveyors on the portal.

04 – Application for fresh license

Q. no. 10 - How to apply for fresh surveyor license:

Ans. – Steps for applying for fresh license:

1. Click Trainee tab

2. Click Licensing tab.

3. Click Fresh License. The Fresh License Application (Form 1-AF) will open. All
the mandatory fields in the form are marked with an asterisk (*).

The Fresh License application process comprises of the following sections:

1. Trainee Details
2. Training and Experience
3. Employee Details
4. License Details
5. Upload Documents
6. Form Preview
The buttons available in the Fresh License Application (Form 1-AF) are
explained in the following table.

Button Description
Click to submit the completed application form once
Submit
you have ticked the declaration check box.
Click to stop the application processing and return to
Cancel the home page. This action will also clear all the
information entered without saving it.
Click to save all the data entered in the form. You can
Save as Draft use this option if you want to submit the form at a later
time.
View Submitted Click to view the submitted Fresh License Application
Form (Form 1-AF) in read only mode.
Click to print the submitted Fresh License Application
Print
(Form 1-AF).
Clear Click to clear the data entered.

The Licensing tab will appear/activate only after successful submission of Training
Completion Certificate by your trainer(s).

Checklist of documents needs to be submitted at the time of filing the online


application:- (fee & some requirements are amended as per IRDAI ( Surveyor)
Regulation 2015)
1. In case opted for offline payment of fees by Demand Draft (DD), DD for Rs.
1000/- in favor of Insurance Regulatory And Development Authority of India,
payable at New Delhi. In case fee paid online, proof of online payment of fees.

2. Copy of Degree/Diploma attested by Notary/Magistrate only.


3. An affidavit on Rs.10/- Stamp Paper as per the format given against Sl. No.2 of
the application from IRDA-1-AF duly notarized (including all the 6 clauses) may
be sent to this office.
4. Self-addressed envelope of 4.5”X10” (As regards amount of stamp for speed
post, please check with postal department and affix stamp accordingly).
5. The Form 1-AF placed on our website www.irda.gov.in may be filled in duly,
completed and sent to this office.
6. No Objection Certificate from your employer, if applicable.
7. Training reports for the all 4 quarter in the prescribed format.
8. Notarized copy of Result Card/Mark sheet issued by the Insurance Institute of
India.
9. The Training Completion Certificate duly stamped and signed by trainer Copy of
PAN Card.
10. Copy of Enrollment letter.
11. Address proof (driving license, passport, etc.)
12. Copy of IIISLA (Indian Institute of Insurance Surveyors And Loss Assessors)
Membership.

05 – Application for renewal of surveyor license

Q. no. 11 – how to apply for renewal of surveyor license?


Ans.- 1. Click Licensing tab.
2. Click Renew License. The Renew License Application (Form
5-AF) is displayed. All the mandatory fields in the form are mark with
an asterisk (*).
3. Late fee will be applicable if applicant has not filed the renewal
application before 30 days from the date of expiry of his/her surveyor
license (eg. - If surveyor license is expiring on 01.07.2015 in that
case he/she should apply before 31.05.2015 otherwise late fee will
be imposed)

The buttons available in the Renewal of License Application (Form 5-AF) are
explained in the following table.

1. In case opted for offline payment of fees by Demand Draft(DD), DD for Rs. 100/-
(in favor of Insurance Regulatory And Development Authority, payable at New
Delhi. In case fee paid online, proof of online payment of fees.
2. With late fee, Demand Draft for Rs. 850 /- (Rs. 750/- if late fee applicable + Rs.
100/- renewal fee). In case fee paid online, proof of online payment of fees.
3. Copy of Degree/Diploma attested by Notary/Magistrate only.
4. An affidavit on Rs.10/- Stamp Paper as per the format given against Sl. No.2 of
the application from IRDA-5-AF duly notarized (including all the 6 clauses) may
be sent to this office.

6. The Form 5-AF placed on our website www.irda.gov.in may be filled in duly,

9. Copy of PAN Card.
10. Address proof (driving license, passport, etc.)
11. Copy of IIISLA (Indian Institute of Insurance Surveyors And Loss Assessors)

06 – Application for modification in surveyor license

Q. no. 12 – How to apply for modifications in surveyor license:-


Ans.- Modification can be made in following items after clicking on link surveyor-
individual followed by licensing:- a. Additional department, b. correction of name, c.
change of address, d. incorporation of sole proprietor. The approval of application may
be given after verification of relevant documents pertains to modification application.

Check lists for modifications:-

a. Additional department – current surveyor license (surrender original surveyor


license), Notarized copy of qualification certificate, notarized copy of III mark
sheet, enrollment letter, training completion certificate, copy of trainer’s license,
copy of detailed training quarterly report for applied department, passport size
photograph 2 psc and self addressed envelope affixed with Rs. 40 stamp .
b. Correction of name - current surveyor license(surrender original surveyor
license), govt approved photo id, affidavit, paper cutting in case of name change,
passport size photograph 2 psc and self addressed envelope affixed with Rs. 40
stamp.
c. Change of address – Current surveyor license(surrender original surveyor
license), proof of change of address (rent agreement, passport, adhaar card,
passbook of nationalized banks, water bill, electricity bill must be issued in the
name of applicant), passport size photograph 2 psc and self addressed envelope
affixed with Rs. 40 stamp.
d. Incorporation of sole proprietor – Current surveyor license(surrender original
surveyor license), certificate of filed service tax/income tax in the name of
proprietor firm, passport size photograph 2 psc and self addressed envelope
affixed with Rs. 40 stamp

07 – Fresh corporate license

Q. no. 13 – what is the procedure for obtaining fresh corporate surveyor license?

1. Users can create user id and password by themselves.


2. Update profile
3. Apply for fresh corporate surveyor license and submit the documents (checklist)
mentioned below:- (fee & some requirements are amended as per IRDAI (
Surveyor and Loss Assessors) Regulation 2015)
a. Duly filled FORM – 3AF in all respect
b. Fit and proper declaration and undertaking to be submitted by all individual
partner/director of applicant company/firm as per Schedule I, Annexure 2 of
IRDAI (Insurance Surveyors & loss Assessors) Regulation, 2015
c. In case opted for offline payment of fees by Demand Draft (DD), DD for Rs.
1000/- in favor of Insurance Regulatory And Development Authority of India,
payable at New Delhi. In case fee paid online, proof of online payment of
fees.
d. Copy of individual surveyor licenses for all partner/Directors
e. Notarized copy of educational qualification of all individual surveyor license
(partner/Directors)
f. Copy of IIISLA membership certificate all individual surveyor license
(partner/Directors)
g. Copy of pan card of all individual surveyor license (partner/Directors)
h. Copy of pan card issued in company’s name
i. Affidavit in original made by company (partner/Directors) as mentioned in
FORM – 3AF , point no. 2
j. Affidavit in original made by all individual surveyor license (partner/Directors)
declaring name, address, SLA number and category and declaration as
mentioned below:-
 Have not been found to be unsound mind by court of law
 Have not found guilty of criminal misappropriation or criminal breach of
trust, or cheating of forgery, or an abetment of or attempt to commit any
such offence by court of Law
 Have not been found guilty of or to have knowingly participated in or
convicted at any fraud, dishonesty or misrepresentation against an insurer
or an insured in the course of any judicial proceedings relating to any
policy of insurance or the winding up of an insurance company or in the
course of an investigation of the affairs of an insurers.

 Will work only under the license of FIRM/company
 Shall not violate the code of conduct specified by the regulations made by
the Authority
 Possess the requisite qualification and practical training as specified by
the regulations made by the Authority.
 Have passed such examination as specified by the regulations made by
the Authority

m. MOM and MOA in case of submission of ROC

08 – Renewal of corporate license

Q. no. 14 - what are the procedures for corporate license:-

1. User id and password can be provided from IRDAI on a request e-mail to


[email protected] subject to production of relevant documents
2. Log in with user id and password on www.irdabap.org.in
3. Update your profile
4. Click on “surveyor-corporate” link
5. Click on licensing
6. Click on renewal of surveyor license
7. Apply for renewal and upload and send hard copies of following documents to
Delhi regional Office for processing:- (fee & some requirements are amended
as per IRDAI ( Surveyor) Regulation 2015)
a. Duly filled Form-6AF
b. Fit and proper declaration and undertaking to be submitted by all individual
partner/director of applicant company/firm as per Schedule I, Annexure 2 of
IRDAI (Insurance Surveyors & loss Assessors) Regulation, 2015
c. In case opted for offline payment of fees by Demand Draft (DD) in favor of
Insurance Regulatory and Development Authority of India, payable at New
Delhi, DD for prescribed fee Rs. 100/ - or Rs. 850 /-(Rs. 100 /- or Rs. 100 +
Rs. 750/-(late fee in case of submission of application within 1 month from the
date of expiry – please refer to individual surveyor license renewal point no. 3
for clarification. In case fee paid online, proof of online payment of fees.
d. Copy of all individual surveyor license (partner/Directors)
e. Notarized copy of educational qualification of all individual surveyor license
(partner/Directors)
f. Copy of IIISLA membership certificate all individual surveyor license
(partner/Directors)
g. Copy of pan card of all individual surveyor license (partner/Directors)
h. Copy of pan card issued in company’s name
i. Affidavit in original made by company (partner/Directors) as mentioned in
FORM – 6AF , point no. 4
j. Affidavit in original made by all individual surveyor license (partner/Directors)
declaring name, address, SLA number and category and declaration as
mentioned in point no. 6 of FORM-6AF

09 – Duplicate surveyor license

Q. no. 15 - Where & how to apply for duplicate surveyor license?


Ans. – www.irdabap.org.in, you can apply with the user id and password provided to
you by IRDAI and upload the and after successfully submission of your online
application send the hard copies of following documents IRDAI Delhi office:-

1. Copy of Valid surveyor license


2. Valid IIISLA membership certificate
3. Complete set of documents as mentioned for fresh surveyor license
4. Copy of FIR for the lost of original valid surveyor license
5. Two Passport size photograph
6. Self address envelope affixed with stamp of amount Rs. 40/-

10 – Lapsed license

Q. no. 16 – how to file the application for fresh surveyor license against lapsed
surveyor license.
Ans. – Lapsed means – A surveyor license which has not been renewed within
prescribed time limit within 6 months from the date of expiry of license. An application
from such surveyors will be treated as fresh application. The Authority will grant the
license with membership level “Licentiate/Associate/fellow” and eligibility of department
may be decided on the basis of qualifications as prescribed in Schedule I, Annexure 1
of IRDAI (Insurance Surveyors & loss Assessors) Regulation, 2015 on qualification
criteria licensing of surveyor and loss assessors.. Application may be filed in offline
mode with the documents mentioned below:- (fee, membership level & qualification
requirements are amended as per IRDAI ( Surveyor) Regulation 2015)
1. Demand Draft for Rs. 1000/- in favor of Insurance Regulatory And Development
Authority, payable at New Delhi.
2. Copy of Degree/Diploma attested by Notary/Magistrate only.
3. An affidavit on Rs.10/- Stamp Paper as per the format given against Sl. No.3 of
the application from IRDA-1-AF duly notarized (including all the 6 clauses) may
be sent to this office.
4. Additional affidavit should be made mentioning following:-
a. There is no disqualification to act as surveyor and loss assessors
b. There is no disqualification as provided under Section 42D of the Insurance
Act from the date of expiry of license SLA…………(surveyor license number)
to till date
5. Self-addressed envelope of 4.5”X10” (As regards amount of stamp for speed
post, please check with postal department and affix stamp accordingly).
6. The Form 1-AF placed on our website www.irda.gov.in may be filled in duly,
completed and sent to this office.
7. No Objection Certificate from your employer for grant of license if applicable.
8. Copy of PAN Card.
9. Address proof (driving license, passport, etc.)
10. Copy of IIISLA (Indian Institute of Insurance Surveyors And Loss Assessors)

 Original license(lapsed) to be surrender with documents

1. The FAQs are indicative Answers. Users are advised to refer IRDAI ( Insurance
Surveyor and Loss Assessors) Regulation 2015. No Inference which is contrary
