    Perez 4109 Trece Martires City-Cavite under oath,depose and state:

    1. I am the and duly authorize representative of Pevero Trading Company Ltd


    (Corporation) a corporation duly organized and existing under and by virtue of the
    laws of the Philippines, with principal office address at Purok 4 Brgy. Hugo Perez
    4109 Trece Martires City-Cavite.

    2. The Corporation is the consignee is the consignee of the following shipment:

    Commodity : 1x40 CTNR stc Stainless Steel Access.


    Bill of Lading No : HLCUGOA160327172
    Consignee : Pevero Trading Company Ltd.
    Vessel : OOCL Kuala Lumpur
    Registry No. : RCL0018-16
    Date of Arrival : May 1, 2016

    3. The Corporation filed a request with the Bureau of Customs for the amendment of the
    Inward Foreign Manifest of the above shipment as follows:

    4. The Corporation filed the said request for untagging of abandonment was the delay in
    payment of final duties and taxes due to late remittance from our Principal in Italy.

    5. Should the Commission on Audit (COA) disallow the release of the above shipment,
    the Corporation undertakes to assume full responsibility for whatever liability and/or
    financial obligation it may entail to the Bureau of Customs, the District Collector and
    the officers/employees of the manila International Container Port.
