EMPLOYMENT CONTRACT
KNOW ALL MEN BY THESE PRESENTS:
I Elaine T. Lantud Filipino, of the legal age, single/married and a resident of

willingly enter into this contract for project
employment with NC APOSTOL INSTITUTE OF TECHNOLOGY INC., a corporation
duly organized and
existing in accordance with laws of the Republic of the Philippines, with principal office
at 3rd Floor
Gesele Park Bldg. Corner EdsaRotonda Pasay City, Philippines.
I agree to be bound by the provisions of this contract as enumerated here under:
1. That my employment is on a project basis withNC APOSTOL INSTITUTE OF

TECHNOLOGY,INC.,
Shall commence on ___________________________ estimated to
last up to
_______________________, and shall be assigned as
_____________________. That, my
appointment is co-terminus, arising from the nature of the project
agreement with the
client. My duties as a/an shall include the following:
a. Performance of duties assigned to me by STEPs
supervisor/coordinator or other representative; and
b. Others (see attached job description)
2. That, for a just cause such as malfeasance, misfeasance, and
nonfeasance, or due to my conduct that are detrimental to the material
interest of NC APOSTOL INSTITUTE., its client, and the place of my
assignment; failure to meet the client or due to violation of rules and
regulationsthat I have committed against NC APOSTOL Inc., its client, and
outlets, I may be terminated beforethe expiration of the period here in
stated.
3. That as a project employee, I shall be entitled to a daily wage, where in the
corresponding rate shall be____________ (Php ____________) subject to
mandatory withholding tax, SSS, Pag-Ibig if any and payable every
_________________________ day of the month at the designated
address of company. Should those days fall on Saturday / Sunday /
Holiday / Non banking days, then payroll would be on the next banking day.
That I shall accomplish the timesheet based on my Daily Time Record (DTR),
which should be submitted every 15th / 30th / 31st day of the month, duty
signed by the authorized signatories. This time report shall be the basic of
the monthly salary.
4. That, I agree to post a cash bond in the amount of
________________________PESOS (Php __________), which shall secure
the payment of any and all damages due to EIA, Inc. arising from my fault or
negligence or from my act or omission in the performance or discharge of
my duties.
In addition to the mentioned cash bond, I agree to post a _____________

(Insurance, surely and others) that the client my require.
5. That I shall not be entitled to any compensation of days I do not render

work pursuant to the no work, no pay principle, except when expressly
mandated by law. I shall not perform any work beyond the established
working hours of the clients and or during legal holidays or rest days, unless
instructed to do so in writing by the Company in which event I agree to do
overtime or additional work.
That I shall not perform any work for NC Apostol, Inc. beyond the term of
this contract as stated in Paragraph (1) hereof.
6. That, my work result performance shall be under the Strict Supervision,

Control and make sure that the end result is in accordance with the standard
specified by the client to NC APOSTOL, Inc.
7. That, NC APOSTOL INTITUTE OF TECHNOLOGY INC., has the right to

transfer me from one assignment to another, as well as change my
working hours as the nature of assignment requires, during the term of
this contract provided that the transfer or change shallbe under the same
project.
8. That, if I should be absent for three (3) consecutive working days without
advanced notice or reasonable explanation, NC APOSTOL INSTITUTE OF
TECHNOLOGY, INC. shall consider me AWOL (Absent Without Leave)
which shall constitute a just cause for termination.
9. That, should I decide to terminated my project employment as provided in

paragraph one (1) hereof, I should provide at least thirty (30) calendar
days written notice toNC APOSTOL INSTITUTE OF TECHNOLOGY, INC. of
such intended resignation.
10. That Ihave read and do understand the rules and regulations of NC
APOSTOL,Inc and shall faithfully abide, obey and comply with all the
rules and regulations of NC APOSTOL, Inc. as required, for the efficient
operation of its business and the proper conduct of its employees, as
well as the rules and regulations and policies of the clients/or outlet to which I
am assigned.
11. That, I hereby bind myself to exercise utmost discretion and
maintain highest level of confidentiality, and I shall not use or
disclose to any person, entity any such records, documents or
information covering the business affairs or NC APOSTOL, Inc. and/or its
clients which I have acquired by reason of my service. All such records,
documents and information in my possession shall be immediately turned
over to the NC APOSTOL, Inc./client on any demand or at any time that the
company may so request.
12. That failure to pay for liquidated damages shall entitle NC APOSTOL,
Inc. to offset such damages against any salaries or monetary benefits
due me, and pursue legal action against me if the money due are not
enough to pay for the damage to EIA, Inc.s interest, without
prejudice to NC APOSTOL, Incs withholding my employment clearances.
