Four prior reports were located on the decedent. In 2013 a report was closed with "not substantiated" findings for substance
misuse and "no indicators" for physical injury. The decedent's mother was the alleged perpetrator. In January 2014 the
decedent and his siblings were removed from the mother as the decedent had left the family's home (which at the time was a
hotel) and was found wondering the lobby while the mother was under the influence in her room. It was at this time the
decedent was placed with his father. The dependency case was closed in September 2014, giving full custody to the father
and allowing the mother visitations. Two reports were received following the removal. A report in April 2014 was closed with
"not substantiated" findings for physical injury with the decedent the alleged victim and the father the alleged perpetrator.
Lastly, a report was most recently closed on this family in February 2015 with "no indicators" for physical injury on the
decedent. The alleged perpetrator was unknown however it was suspected the abuse was occurring at the father's residence.
No further prior reports were located on the decedent.
