Collaborative Practice Agreement


This agreement sets forth the terms of the Collaborative Practice Agreement between Alexander
Blaine Warren APRN FNP-C and Craig Bishop D.O. at Valdosta Family Medicine in Valdosta,
Georgia. This agreement shall take effect as of June 1st, 2015.
I. Scope of Practice
The nurse practitioners duties as a registered professional nurse operating as an FNP-C includes
the diagnosis of illness and the attempt to correct these illnesses through measures including the
prescribing of medications if the medications are contained in my authorized scope of practice.
Here in the state of Georgia as a nurse practitioner, by law, can prescribe controlled substances
Schedule III Schedule V under a DEA number. The nurse practitioners scope of practice also
includes disease process/health teaching, counseling of health, and case finding. This practice
will take place at Valdosta Family Medicine or in such other facility or location as designated by
Craig Bishop, D.O. or by the parties of this contract.
II. Practice Protocols
The nurse practitioner is authorized to diagnose and treat common medical conditions under the
following current guidelines (including, but not limited to):

III. Physician Consultation

The signing physicians will be available to the nurse practitioner for consultation, whether
through face to face interaction or through electronic devices. If Craig Bishop D.O. is
unavailable the following physicians will be made available for consult: Stewart Williams M.D.
and Judit Sule M.D.
IV. Record Review
Every three months Craig Bishop D.O. will review a sample of the nurse practitioners records.
This will be done to ensure the protocols, as listed above, are being followed and the nurse
practitioners care is both adequate and within current guidelines. Results of the review will be
discussed with the nurse practitioner and a record of the review will be kept on site.
V. Resolution of Disagreements
Disagreements between Alexander Warren APRN FNP-C and Craig Bishop D.O. regarding the
management of patients care will be resolved after a review of current medical guidelines and
other peer reviewed literature. If the disagreement of care cannot be agreed upon the physicians
opinion will be followed.
VI. Alteration of Agreement
This collaborative agreement will be renewed annually and changes will be made if necessary.
VII. Agreement
After reading the content of this agreement, Craig Bishop D.O. and Alexander Warren FNP-C
agree to the terms as set.
