na rin ako ng gana araw araw Objective: - Fatigue - Restless - Dizziness - Greater need to sleep and rest - VS - BP: - RR: - PR: - TEMP:
Diagnosis Activity intolerance related to imbalance between oxygen supply and demand as evidenced of clients complain of feeling weak
Planning Short term: After 8 hours of nursing interventions, the client will: - Report increase in activity tolerance including activities of daily livings. - Demonstrate a decrease in physiological signs of tolerance Long term: After a month of nursing interventions, the patient: - Is free from weakness and risk for complications has been prevented.
Intervention Indepentdent: - Assess patients ability to perform normal task or activities of daily living. - Note changes in balance ,and muscle weakness.
Rationale influences choice of intervention s or needed assistance may indicate neurological changes associated with vitamin 12 deficiency, affecting patient safety or risk injury enhanced rest to lower bodys oxygen requirement s and reduce strain on the heart and lungs. Enhances lung expansion
Evaluation Patients reveal an increase in activity tolerance, demonstratin g a reduction in physiological signs of intolerance and laboratory values within normal range.
Provide or recommend assistance with activities or ambulation as necessary, allowing patient to do as much as possible. Plan activity progression with patient. Increase level of activities as tolerated.
to maximize oxygen for cellular uptake. Self esteem is enhanced when patient does some things for self.
Identify or implement energy saving techniques like sitting when doing task -
Promotes gradual return to activity level and improve stamina without undue fatigue Encourages patient to do as much as possible, while conserving limited energy and
preventing fatigue Identifies deficiency in RBC components affecting oxygen transport and treatment need or response to therapy.
Objectives After 2 minutes of discussing the definition of anemia, the patient will be able to have knowledge and understanding of anemia.
Learning content Anemia is a condition that occurs when the number of red blood cells (RBCs) and/or the amount of hemoglobin found in the red blood cells drops below normal. Types of anemia: -iron defiency lack of iron to decreased amount of hemoglobin -Vitamin B deficiency does not allow
Rationale For the patient To Enhanced and have a better knowledge about anemia.
Evaluation Was the client able to understand the definition of anemia? YES__ NO__ WHY__________
After 10 minutes of discussing the types and cause of anemia, the patient will be able to have knowledge
