TERMS AND CONDITIONS FOR REIMBURSEMENT

HOW TO CONTACT US
An efficient and caring team is at your disposal to reply to requests for information, payment of healthcare, prior approval or reimbursement of healthcare expenses. Your correspondents have a practical knowledge of foreign social security schemes and are able to process your files in all languages.

At your request, the detailed accounts of reimbursement may be sent by e-mail. All you need is a navigator such as Internet Explorer or Netscape Navigator in a later version than version 3 to send us a written request (stating your GMCI identification number). If you have another type or different version of the navigator, please contact us.

If however, you require information concerning a detailed account of reimbursement, contact your direct and personal correspondent, whose name and contact details appear on the detailed account of reimbursement.

WHAT ARE THE PROCEDURES FOR A REQUEST FOR PRIOR APPROVAL?


Requests for the payment of hospital care (including births), successive medical and surgical procedures, auxiliary medical procedures, dental and non-dental prosthetic appliances, implants, dento-facial orthopaedics, stays in sanatoriums, spa therapies, must be subject to the prior approval of GMCIs Medical Commission, before the care is given. The request for prior approval: standard Social Security form for healthcare in France, GMCIs form for healthcare abroad, must be sent, at least 10 days before the scheduled for the beginning of treatment, in an envelope marked as confidential to : Commission Mdicale de GMCI 10, rue Henner - 75459 PARIS cedex 09 FRANCE It is filled in by the medical practitioner and must include a detailed description of the healthcare together with an exact estimate cost.

HOW TO OBTAIN THE REIMBURSEMENT OF YOUR


HEALTHCARE EXPENSES In order to be reimbursed as rapidly as possible, please send all the following documents to our departments as soon as reasonably possible :

WHAT TO DO IN THE EVENT OF HOSPITALISATION


GMCI only pays hospital costs (including births) directly. Hospitalisation which is clearly urgent You must contact our local correspondents within 3 days of having been admitted. Their contact details appear on your subscribers card. They may be contacted 24 hours per day. The following information should be specified on the REQUEST FOR PRIOR APPROVAL RELATING TO HOSPITALISATION form, available upon request: the name and full details of the healthcare establishment, the exact nature of the treatment, the detailed estimated cost, the arrival and departure dates from the hospital. Following verification to entitlement to the services and the agreement of GMCIs Medical Commission, the case management statement will be sent to be directly regularised at the healthcare establishment. In other cases, notice will be sent to the beneficiary. Non-urgent hospitalisation The procedures are the same as for urgent hospitalisation. However, the "Request for prior approval relating to hospitalisation" form must be sent at least 10 days prior to the scheduled date for the hospitalisation. Where the hospitalisation last for over 30 days, the request for prior approval must be renewed every 30 days. GMCIs Medical Commission must receive it within 10 days.
