FOR ALL UNDER 18s I give full permission for the person named above to take part in this Youth 2000 event. In the event of illness or accident I delegate the organisers or their assistants the authority to authorise in my name any medical treatment that my son/daughter may require. I accept that should my child break the retreat rules, Youth 2000 will not be held responsible for my childs action, and I agree to collect my child from the venue if asked to do so by a member of the Youth 2000 team. I understand that filming and photography may take place during the retreat, and give consent for any footage which includes my son/daughter to be used by Youth 2000. I understand that this event finishes at 6pm prompt and that Youth 2000 and the parish of Our Lady and All Saints, Stourbridge cannot accept any responsibility for my child from 6pm. FOR UNDER 16s ONLY You must be part of a group accompanied by a responsible adult - one such Group Guardian for every five under 16s. Name of group guardian Dear Guardian On behalf of Youth 2000 we thank you for arranging to bring a group of young people with you to our retreat. We hope you all have a blessed time with us and enjoy the prayer festival. As you will appreciate, we need to ensure that we do our utmost to keep all the people joining us for the prayer festival safe at all times. We ask that the following rules be adhered to: Minors cannot be left unsupervised at any time. Youth 2000 adheres to government recommendations that all groups containing persons under the age of 16 taking part in our festival are accompanied by responsible guardians at a ratio of 1 adult to 5 children. Ideally all groups must be accompanied by at least 2 responsible guardians; otherwise, in the event of an emergency (medical or otherwise) that results in one of the minors having to leave the retreat, the remainder of the group cannot be left unsupervised and will have to leave the retreat with the guardian also. In groups of 5 or less if there is only one guardian they must take the whole group off site if they leave. Youth 2000 cannot accept any responsibility or offer supervision for minors at the prayer festival. NB. Every person aged 16+ must agree to the safety guidelines policy which can be found on the Youth 2000 website. This agreement is confirmed by ticking the box on the booking form.
