Incident Report

Question: N level 2006

Yesterday, as you were walking to school, you saw a car go out of control. It was
driven by a learner driver, accompanied by an instructor. The car crashed into a stall
by the roadside. The stall overturned, injuring the stallholder and his daughter.

The Police have asked you to write a report on exactly what you saw.

Your report must include the following details:


• Where and when the accident happened
• What road conditions were like at the time
• A brief description of the car and the driver
• What injuries were caused
• What other damage there was

Your report must be a clearly expressed, factual account and you should include any
further information which you think would be helpful. Remember to give your report
a heading.

to help the Police in their investigations. This is especially if
someone was injured or killed.

The following is a report of the incident which I witnessed outside the school
gate yesterday afternoon (Monday, 23 August 2007) at around 3 pm. I was Brief
walking towards the school gate when I saw three boys lingering outside the description
school gate. Two of the boys, both Chinese, were from Lihua Secondary of the two
School. They were in their school uniforms. One of them was tall and assailants,
which may
heavily built and the other had a small frame and hair which fell to his help to
collar. The third boy was David Ng, a Secondary 2 student from our school.
The two boys from Lihua Secondary School were behaving very aggressively
towards David, shouting and gesturing wildly at him.

Suddenly, I saw David turning to run away but the bigger boy grabbed him Vivid account

Report of accident at Choa Chu Kang Ave 4

On 29 March, at about 6.30 a.m., I witnessed a car accident at Choa Chu


Kang Ave 4, near block 625.

The accident involved a learner driver, an instructor, a stallholder and his


daughter. On that day, there were foreign objects on the road surface.
The street lights were not on. It was drizzling at that time and the road
surface was slippery.

A red Toyota Corolla car with registration number SDQ 8888 U was being
driven by a female Chinese learner driver accompanied by an instructor.
The car skidded and crashed into a stall by the roadside. The stall
overturned, injuring the stallholder and his daughter. The car was
travelling at quite a high speed when the driver braked suddenly.

The stallholder lay on the road unconscious. He suffered head injuries


and was bleeding profusely. His daughter, about 6 years old, had some
bruises on her arms and legs. She looked pale and was in a state of
shock. The car was badly dented in the front and its windscreen was
shattered. Both the instructor and the driver were not injured but
suffered severe shock. Someone in the crowd called the police.

When the ambulance arrived, the injured stallholder and his daughter
were sent to the hospital, while the driver and the instructor stayed
behind to give their statements to the police. Skid marks could be seen
on the road. I was asked to help in the investigations by writing this
report on what I had witnessed.

I hope that my statement would be of help in the investigations and I
