The SpE must be implenrented and compbted m approved by Qgl- tur the Education program. It is me reponsibility of
the student to submit an ApplicaUon for Assessment Extension form for approval and negotiate mutually
acceptable days to make up any absence with their SPE Mentor and the SPE Site Coordinator.
This SpE Student Attendance Form must be signed by aII relerant persons. It 6 drc student's responsibility to
ensure tlrc correct SpE form is signed and returned with ttre SPE Final Report at the end of the Supervised Professional
Experience. Failure to submit thG form on time may lead to a Grade Pending as this form is required for finalisatbn of
achievement of a course assessment.
