    I am requesting funding for Cherokee Court Team for Maltreated Infants and Toddlers. The entity
    to receive funding for this project is Zero to Three, located at P.O. Box 1629 Cherokee, NC 28719.
    The funding would be used for providing assistance to maltreated infants and toddlers, and to help
    families and children in need. I certify that neither I nor my spouse has any f111aneial interest in this
    project.
