Spondyloarthritis

There are no formal statistics about the incidence, prevalence or demographics of patients with spondyloarthropathies (SpAs) in Colombia. However, information from a few studies provides a preliminary snapshot of SpAs in the country. In this article, the authors review what has been published; document what their group is doing and outline what they still need to do in the future. The analysis suggests that although information on SpA in Colombia is limited, it is known that the diagnostic entities of SpA are different than those reported at other latitudes. Thus, it is important to improve and expand the current database of SpA, particularly undifferentiated SpA, not only in Colombia but in all of Latin America.

Background: Ankylosing spondylitis is a relatively common worldwide chronic inflammatory disease that usually affects spine, sacroiliac joints in addition to peripheral joints and affects multiple other tissues, which can progress to bony fusion of the spine.

Objective: To fill the gap of the absence of a published description of this condition in Libyan patients and to compare of the clinical presentation and other aspect of the condition in a Libyan patient with what is known worldwide.

Materials and Methods: A case report of a 60 year old Libyan male who has Ankylosing spondylitis and then review the available literature.

Results: The patient has been having a chronic low back pain and stiffness for the last 26 years. His symptoms are most severe in morning and improve with movement. He developed stiffness and limitation of movement of the neck and restriction of movements of the shoulder and hips. Examination showed kyphosis and limited neck movement and decreased chest expansion. His score on Bath Ankylosing Spondylitis Disease Activity Index is 6. His x-rays showed bamboo spine appearance of spine and osteoarthritis in both shoulder joints and hip joints. His presentation fits both European Spondyloarthropathy Study Group diagnostic criteria for spondyloarthropathies and the modified New York Criteria.

Conclusion: Our Libyan patient shows a classical clinical and radiological picture of the condition and the diagnostic criteria and severity index could be applied easily to his cases.

This preliminary study aims to describe the HLA-B27 allele frequency in Sri Lankan patients with spondylo-arthritides (SA). An anonymised database of 373 Sri Lankan patients with SA referred for HLA-B27 testing was retrospectively analysed. Eighty five (22.8%) patients were positive for the HLA-B27 allele. A male preponderance was observed among the positives. The HLA-B27 allele frequency in this sample of patients with SA was relatively low compared to published studies in other populations. Further research is needed to identify the predominant subtypes of the allele to determine which subtypes are the most prevalent in a larger sample of Sri Lankan patients with SA, and to define their association with the specific types of SA.

Background Dysbiosis has been recently demonstrated in patients with ankylosing spondylitis (AS) but its implications in the modulation of intestinal immune responses have never been studied. The aim of this study was to investigate the role of ileal bacteria in modulating local and systemic immune responses in AS. Methods Ileal biopsies were obtained from 50 HLA-B27 + patients with AS and 20 normal subjects. Silver stain was used to visualise bacteria. Ileal expression of tight and adherens junction proteins was investigated by TaqMan real-time (RT)-PCR and immunohistochemistry. Serum levels of lipopolysaccharide (LPS), LPS-binding protein (LPS-BP), intestinal fatty acid-BP (iFABP) and zonulin were assayed by ELISA. Monocyte immunological functions were studied in in vitro experiments. In addition the effects of antibiotics on tight junctions in human leukocyte antigen (HLA)-B27 transgenic (TG) rats were assessed. Results Adherent and invasive bacteria were observed in the gut of patients with AS with the bacterial scores significantly correlated with gut inflammation. Impairment of the gut vascular barrier (GVB) was also present in AS, accompanied by significant upregulation of zonulin, and associated with high serum levels of LPS, LPS-BP, iFABP and zonulin. In in vitro studies zonulin altered endothelial tight junctions while its epithelial release was modulated by isolated AS ileal bacteria. AS circulating monocytes displayed an anergic phenotype partially restored by ex vivo stimulation with LPS+sCD14 and their stimulation with recombinant zonulin induced a clear M2 phenotype. Antibiotics restored tight junction function in HLA-B27 TG rats. Conclusions Bacterial ileitis, increased zonulin expression and damaged intestinal mucosal barrier and GVB, characterises the gut of patients with AS and are associated with increased blood levels of zonulin, and bacterial products. Bacterial products and zonulin influence monocyte behaviour.

Psoriatic arthritis (PsA) is an archetypal type of spondyloarthritis, but may have some features of rheumatoid arthritis, namely a small joint polyarthritis pattern. Most of these features are well demonstrated on imaging, and as a result, imaging has helped us to better understand the pathophysiology of PsA. Although the unique changes of PsA such as the "pencil-in-cup" deformities and periostitis are commonly shown on conventional radiography, PsA affects all areas of joints, with enthesitis being the predominant pathology. Imaging, especially magnetic resonance imaging (MRI) and ultrasonography, has allowed us to explain the relationships between enthesitis, synovitis (or the synovio-entheseal complex) and osteitis or bone oedema in PsA. Histological studies have complemented the imaging findings, and have corroborated the MRI changes seen in the skin and nails in PsA. The advancement in imaging technology such as high-resolution "microscopy" MRI and whole-body MRI, and improved protocols such as ultrashort echo time, will further enhance our understanding of the disease mechanisms. The ability to demonstrate very early pre-clinical changes as shown by ultrasonography and bone scintigraphy may eventually provide a basis for screening for disease and will further improve the understanding of the link between skin and joint disease.

o report adalimumab (Ada) efficacy on articulargastrointestinal disease and health-related quality of life (HRQoL) in patients with enteropathic spondyloarthritis(ES). METHODS A cohort of 52 patients with ES was evaluated in the departments of gastroenterology and internal medicine. At baseline, all patients underwent assessment by an integrated gastro-rheumatologic evaluation of articular and gastrointestinal activity, as well patient reported outcomes (PROs) of the HRQoL questionnaires. After this integrated evaluation and following a specific working flowchart, the Ada anti-tumor necrosis factor (TNF)-inhibitor was assigned to a cohort of 30 patients and its clinical efficacy was evaluated at baseline and after 6-mo and 12-mo treatment by the following tests: (1) Ankylosing Spondylitis Disease Activity Score- C-Reactive Protein (ASDAS-CRP); Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Bath Ankylosing Spondylitis Functional Index (BASFI) and Bath Ankylosing Spondylitis Metrology Index (BASMI) for articular activity; (2) Inflammatory Bowel Disease Questionnaire (IBDQ), Crohn’s Disease Activity Index (CDAI) and partial Mayo (pMayo) score for gastrointestinal symptoms and activity; and (3) Health Assessment Questionnaire (HAQ), Patient Global Assessment (PGA) and Short Form-36 health survey (SF-36) questionnaires for PROs of the HRQoL. RESULTS Integrated evaluation and management of the patients affected by ES, carried out simultaneously by a gastroenterologist and a rheumatologist, allowed clinicians to choose the optimal therapeutic strategy. In a cohort of 30 ES patients affected by active articular and gastrointestinal disease, or axial active articular inflammation, Ada led to fast and sustained improvement of both articular and gastrointestinal disease activities. In fact, all the clinimetric evaluation tests exploring articular or gastrointestinal activity, as well as all the HRQoL scores, showed a significant improvement having been achieved at the earliest (6-mo) assessment. This important clinical improvement was maintained at the 12-mo follow-up. Importantly, global and gastrointestinal quality of life significantly correlated with articular disease activity, providing evidence to support that the integrated evaluation is the best option to manage patients with ES. CONCLUSION Ada treatment, upon multidisciplinary (gastrorheumatologic) evaluation, significantly improves both articular and gastrointestinal inflammation, thereby improving the HRQoL in patients affected by ES.

Les maladies app artenant au groupe des spondylarthropathies inflammatoires ont en commun, outre des facteurs génétiques et certaines caractéristiques cliniques, une cible pathologique particulière : l’enthèse. Les données médicales récentes tendent à prouver que les enthésopathies des spondylarthropathies inflammatoires divergeraient de celles causées par les processus dégénératifs en se caractérisant par des lésions érosives aboutissant secondairement à une ossification. Afin d’évaluer la validité de cette hyp othèse sur le matériel ostéologique, une étude comparative entre les lésions des enthèses fibrocartilagineuses app endiculaires de six individus atteints de spondylarthropathies et d’individus non affectés par la maladie provenant d’un même échantillon a été réalisée, en tenant compte de l’influence du vieillissement sur les signes osseux d’enthésopathies. Les résultats indiquent que des ostéolyses, associées ou non à une production osseuse cicatricielle, sont les remodelages majoritairement observés pour certaines insertions des individus malades, alors que cet aspect est plus rare dans le reste de la population. Ces résultats concordent avec les données de la littérature médicale et permettent de discuter des spécificités du remodelage des enthèses fibrocartilagineuses dans les spondylarthropathies inflammatoires diagnostiquées sur du matériel osseux.

The spondyloarthropathies are a group of related conditions which have in common genetic factors, clinical features and a specific pathological target: the enthesis. Recent medical data point out the enthesopathies in spondyloarthropathies would differ from those caused by degenerative processes, being characterized by erosive lesions leading secondarily to an ossification. In order to evaluate the validity of these assumptions on the osteological material, a comparative study between the appendicular fibrocartilaginous entheses lesions of six individuals showing signs of spondyloarthropathies and those of “healthy” individuals coming from the same sample was realized. The influence of ageing on the osseous signs of enthesopathies was also considered. The results indicate that osteolysis, sometimes associated with a cicatricial osseous production, is the mainly observed remodelling aspect for some insertions of the six cases, whereas this feature is rare in the remainder of the population. These results agree with the medical literature data and make it possible to propose a first characterization of the remodelling process of entheses in spondyloarthropathies diagnosed on osteological material.

Objective: Seronegative spondyloarthritis (SpA) is characterized by chronic inflammation affecting the axial skeleton, entheses and occasionally peripheral joints. The involvement of the sacroiliac joints, sacroiliitis, is considered as a pathognomonic radiographic finding. Magnetic resonance imaging (MRI) is the method of choice for its early detection. Bone scintigraphy (BS) is characterized by high sensitivity in the diagnosis of bone and articular diseases. Limited value of BS in the diagnosis of sacroiliitis may be attributed to the use of planar imaging. In the present study, we aimed to investigate the role of SPECT in SpA, compared to MRI. Methods: Forty-three patients suffering from inflammatory back pain underwent MRI evaluation of the sacroiliac joints and BS, combined with SPECT in the same region, for the assessment of sacroiliitis. Results: Bone SPECT revealed no findings of sacroiliitis in 11 patients, with total agreement with MRI. Findings of chronic lesions were demonstrated from both modalities in 2 patients. Bone SPECT and MRI findings were in concordance regarding the investigation of active sacroiliitis, with the exception of one patient with mild SPECT findings and negative MRI examination; the diagnosis of AS however, was established one year later, after a positive follow-up MRI. The evaluation of the planar imaging of the whole skeleton and SPECT imaging, revealed additional lesions. Conclusions: Bone SPECT is a reliable imaging method in the diagnosis of active sacroiliitis. Its application on planar BS, an economic and widely available diagnostic technique, appears to be a valuable aid for the clinician. Kemik sintigrafisi (KS) kemik ve eklem hastalığının tanısında yüksek duyarlılığa sahiptir. KS'nin sakroiliit tanısındaki kısıtlı değeri planar görüntüleme kullanılmasına bağlı olabilir. Bu çalışmada SpA'da SPECT'in rolünün MRG ile karşılaştırılması amaçlandı.

Military personnel are often subjected to physical exertion, sleep deprivation, deficient diets, overcrowding, and stress. All of these influences are capable of compromising the immune system’s ability to ward off disease-causing bacteria, thus explaining why the historical narrative of war is frequently accompanied by reports of death and suffering due to epidemics of infectious diseases. Historically some of the most common infections included: diarrhoea, dysentery, typhoid fever, gonorrhoea, and streptococcal tonsillitis. The bacteria which cause these diseases are also capable of triggering arthritis. When an arthritic condition is triggered by an infectious microbe it can broadly be referred to as “reactive arthropathy,” of which the spondyloarthritides (SpAs) are of great interest. Since the bacteria associated with these arthritic conditions are responsible for the epidemics which have plagued combatants for centuries, it is reasonable to assume that reactive arthropathy was present in past military populations. This assertion can be tested through a prevalence study of military related skeletal assemblages. To test the methodology and gain preliminary results for this research project, a pilot study was carried out using remains from the 1461 Battle of Towton. The methodology was deemed to be sound and the statistical results, while not significant, were promising.
