Background-Financial toxicity describes the financial burden imposed onto patients by a cancer diagnosis and is a growing concern. Many clinicians do not currently address financial toxicity despite patients' desire for them to do so. Current literature explores physicians' perspectives but does not clearly define an actionable role clinicians can take to address financial toxicity. We sought to fill this gap by first assessing clinicians' perspective on their role in alleviating financial toxicity at our institution. We subsequently aimed to identify current barriers to mitigating financial toxicity and to garner feedback on clinician-oriented interventions to address this growing problem. Methods-We developed an 18-item electronic, anonymous survey through Redcap. We invited all oncology clinicians including attending physicians, advance practice providers, and trainees at our institution to participate. Results-A total of 72 clinicians (30%) completed the survey. The majority agreed that clinicians have a role in addressing cost. The top three barriers to discussing cost with patients were knowledge of out of pocket costs, time, and awareness of resources. Editor-Although I agree with Empey that the recent direction from the Department of Health on the suspension of doctors represents progress-major progress with the mandatory need for referral to the National Clinical Assessment Authority (NCAA)-he has glossed over the fact that it has taken nine years for it to be produced. 1 He also failed to highlight the risk to patients' safety that is caused by suspensions and threats of suspensions, as exemplified by case 8 in the National Audit Office's report, in which a trust's culture was such that consultants feared bullying and victimisation if they raised any concerns about safety issues. 2 Badly planned and executed suspensions compromise care for patients and put them at risk; in view of the requirements of the October 2002 Code of Conduct for Managers to give top priority to protecting patients from risk the rash, rushed, arbitrary suspension of a doctor should not be happening these days. If it does then the responsible manager should be held to account by either the trust board or even the NHS chief executive under the terms of the code. If those pathways of accountability fail then MPs on the Committee of Public Accounts can hold the Department of Health to account on behalf of the public. The procedure and processes around suspensions of both senior managers and consultants are complex and require specialist human resources and legal skills and good, non-biased judgment to avoid unfair dismissal and inappropriate suspensions. A case could be made for establishing a central unit. Alternatively a more loose network of appropriate legal and human resources experts could be established to be called on by any trust to help in these difficult cases so that the proper processes are followed.

Objective: Social recovery and external money management are important approaches in contemporary mental health care, but little research has been done on the relationship between the two or on application of recovery principles to money management for people at risk of being assigned a representative payee or conservator. Method: Out of 49 total qualitative interviews, 25 transcripts with persons receiving Social Security insurance or Social Security disability insurance who were at risk of being assigned a money manager were analyzed to assess the presence of recognized recovery themes. Results: The recovery principles of self-direction and responsibility were strong themes in participant comments related to money management. Conclusions and Implications for Practice: Money management interventions should incorporate peoples' recovery-related motivations to acquire financial management skills as a means to direct and assume responsibility for one's finances. Staff involved in money management should receive training to support client's recovery-related goals.
