This review aims to discuss the latest recommendations regarding diagnosis and treatment of NAFLD-associated HCC and the remaining challenges. Thus, there is a high amount of data derived from basic and clinical studies that revealed the molecular pathways which drive NAFLD-associated hepatocellular carcinoma (HCC). Based on these findings, new prevention, surveillance and treatment strategies are emerging. The aim of this study was to describe the anatomical variations in the internal carotid artery in relationship to sphenoidal sinuses, using computed tomography (CT). Materials and Methods: In this retrospective study, we evaluated the variations of the ICA in relationship to sphenoidal sinuses in a cohort of 600 patients who were assessed between January 2020 and December 2022 in ‘Saint Spiridon’ Emergency Hospital, Iasi, Romania. Introducere: Carcinoamele periampulare reprezintă un grup de tumori ce se dezvoltă într-o regiune... more Introducere: Carcinoamele periampulare reprezintă un grup de tumori ce se dezvoltă într-o regiune complexă, formată din diferite structuri anatomice. Cel mai frecvent tip histologic al carcinoamelor periampulare este adenocarcinomul. Adenocarcinoamele periampulare de tip pancreatic au cel mai sumbru prognostic. Merkerii imunohistochimici, precum ki-67 şi p53, pot fi utilizaţi în predicţia supravieţuirii. Material şi metodă: au fost selectaţi pacienţii cu adenocarcinoame periampulare de tip intestinal sau biliopancreatic, cu tumori rezecabile şi au fost realizate marcaje imunohistochimice pentru markerii ki-67 şi p53. Supravieţuirea a fost analizată în funcţie de expresia markerilor imunohistochimici, stadializarea TNM, diferenţierea tumorală şi invazia perineurală. Rezultate: 67 de pacienţi au fost incluşi în studiu. Mediana supravieţuirii pe întreg lotul de studiu a fost de 12 luni, cu o rată de supravieţuire la 2 ani de 25%. Rata de proliferare tumorală crescută (ki-67 peste 80%) a avut o asociere semnificativă cu scăderea supravieţuirii (o mediană a supravieţuirii de 3 luni versus 17 luni pentru grupul de pacienţi cu ki-67 mai mic de 80%). Creşterea expresiei proteinei p53 s-a asociat cu o scădere a supravieţuirii. Supraieţuirea scăzută s-a asociat cu tumorile slab diferenţiate şi statusul limfoganglionar.

The presence of peritumoral fibrosis is considered the protective factor for the development of postoperative pancreatic fistulas after pancreatic resections for pancreatic ductal adenocarcinomas. Methods: We analyzed a series of 109 consecutive patients with pancreatic resections for malignant pathology: pancreatic ductal adenocarcinomas and periampullary adenocarcinomas. Methods: CT examination was performed in 56 patients with HCV-related compensated cirrhosis before and within 6–18 months after the treatment with Ombitasvir/Paritaprevir/ritonavir + Dasabuvir. Methods: We reviewed the medical records of the patients from our clinic with abdominopelvic actinomycosis who underwent surgery between 2002 and 2022. In this period, 28 cases (9 men and 19 women) were treated. The mean age was 43.36 years and they were hospitalized for abdominopelvic tumors or inflammatory tumors in 15 cases and inflammatory disease in 13 cases. Results: Causes of actinomycosis in the studied group were an intra-uterine contraceptive device in 17 cases, foreign bodies in 2 cases, diabetes in 4 cases, stenting of the bile duct in 1 case, and immunodepression. Hepatic artery anatomic variants (HAav) are frequently encountered during PD. Aim To evaluate the incidence of HAav in a series of PDs and their consequence on technical tailoring and outcome. Material and Methods Forty-five patients with HAav were identified in a consecutive series of 140 PDs for periampullary and pancreatic head tumors performed between January 1, 2007 and December 31, 2015. The groups with or without HAav were compared in terms of operative approach, complications and survival. Results All patients underwent Whipple procedure, with right posterior approach for those having HAav. HAav was spared without damage in 41 cases. However, the procedure is also recommended in some benign pancreatic tumors. The posterior approach allows early dissection of the superior mesenteric artery, portal vein and retroportal pancreatic lamina, before any pancreatic or digestive transection. We present a 42 year old woman diagnosed with a pancreatic tumor. The clinical and biological data suggested the diagnosis of insulinoma. The computed tomography showed a nodule located in the pancreatic head with a typical vascular pattern for endocrine tumor. The exam also revealed a rare vascular variant, a common hepatic artery which arises from the superior mesenteric artery. A pancreaticoduodenectomy has been performed. Our study aims to assess the outcome of large adrenal tumours laparoscopically resected. All patients with laparoscopic adrenalectomy performed in between 2002 and 2009, without preoperative or intraoperative malignant characteristics, were reviewed. Clinical, biochemical and CT follow-up data were reviewed for evidence of recurrent disease. Fifty patients underwent laparoscopic adrenalectomies in our unit, 18 of them having solid cortical tumours > or = 7 cm without preoperative or intraoperative malignant features: 6 Cushing's syndrome tumours, 8 non-secreting tumours, 4 aldosteronomas. Recent improvements in surgical techniques allowed palliative gastric resection and other surgical procedures. We have examined the place of palliative gastrectomy and its impact on survival in stage IV gastric cancer patients admitted in 2003-2008 period. From a total of 295 patients with gastric cancer, we found 140 patients with stage IV disease; 85 of them had no resection (45 received only chemotherapy) and 55 underwent palliative gastric resection with or without postoperative chemotherapy. Mean survival in non-operated patients with chemotherapy alone was 6.4 months, not significantly different to that of the patients with palliative surgery alone (8.9 months). The group with palliative surgery and adjuvant therapy had a significantly better mean survival (17.8 months). Patients succumbing to carbon monoxide poisoning are usually considered unsuitable for organ procurement and they are routinely rejected in many transplant centers. As the organ supply is by far insufficient to meet the needs of patients from the waiting lists for transplant, there is a greater effort to increase the donor organ pool. Therefore special attention is being devoted to such "marginal" or "high-risk "donors. Similarly, tumors revealed during organ procurement require careful consideration in the evaluation of organ donors and transplant recipients. We herein report a multiorgan procurement from a victim of carbon monoxide poisoning who underwent a prolonged cardiopulmonary resuscitation and was intraoperatively diagnosed with a right colon tumor. Intraoperative frozen section examination ruled out malignancy and revealed an acute ischemic colitis. It is suggested that patients dying of carbon monoxide poisoning who are considered appropriate donors, may have intraoperatively revealed pseudotumoral ischemic colitis which does not preclude multiorgan procurement.

Though, for a statistical model, analysis of the regression parameters via confidence intervals, significance tests, and other tests can be used to evaluate the model’s legitimacy. To demonstrate the applications and utility of this theory, we performed an experimental study on zebrafish exposed to the mycotoxin. Methods: Patulin (70 µg/L) and kojic acid (100 mg/L, 204 mg/L, and 284 mg/L) were administered by immersion to zebrafish once daily for 7 days period before the behavior testing. The following behavioral tests were performed: Novel Tank test (NTT) (to assess the explorative behavior and anxiety); Y Maze test (which measures the spontaneous explorative behavior). Behavioral tests were performed on separate days. For the behavior tests, the statistical analysis was performed using ANOVA variation analysis (two-way ANOVA). All results are expressed as the mean ± standard error of the mean. The values of the general index F for which p <0.05 were considered statistically significant. Results: Y-maze - Patulin exposure led to an intensification of the locomotor activity, an increased traveled distance and number of arm entries. By increasing the spontaneous alternation between the aquarium’s arms, Patulin has shown a stimulating effect on spatial memory. In the case of zebrafish exposed to 100 mg /l kojic acid, the traveled distance was shorter by 27% than the distance attained by those in the control group. The higher doses of kojic acid (204 mg / l and 284 mg / l) lead to an increased locomotor activity, distance traveled, number of arm entries and the spontaneous alternation. The increase in spontaneous alternation demonstrates that 204 mg/l and 284 mg/l kojic acid doses had a stimulating effect on spatial memory. Novel Tank test - Compared to the control group, the traveled distance of the patulin exposed fish is slightly reduced. Compared to the control group, the traveled distance of the kojic acid exposed fish is reduced, due to a shorter mobile time (by 25-27% in the case of fish exposed to 204 mg / L and 284 mg / L kojic acid). Patulin and kojic acid exhibits toxic effects on zebrafish liver, kidney, myocardium and leads to severe alteration. We continued the analysis trying some machine learning algorithms on the classification problems in the case of the two behavioral tests MAZE and NTT, after which we concluded that the results were better in the case of the NTT test relative to the MAZE test and that the use of decision tree algorithms leads to amazing results, knowing that their hierarchical structure allows them to learn signals from both classes. Conclusions: The groups exposed to patulin and kojic acid show histological changes in the liver, kidneys, and myocardial muscle tissue. The novel tank test, which assesses the explorative behavior, proved to be conclusive in the behavioral analysis of fish that were given toxins, proving that intoxicated fish had a decreased explorative behavior and an increased anxiety. We have managed to detect an automatic learning algorithm, from the category of decision trees, which can be trained to classify the behavior of fish that were administered a toxin from the category of those used in the experiment, only by analyzing the characteristic features of the NTT behavioral test.
