Anniversary of me taking this pic of my kid’s food and sending it to my medical friends “what the hell is this?” Rash on bottom lateral aspect of foot, looks like a contact
In another few weeks it would look like this, and the second kid came to me with similar but milder toe findings, and we all figured out we’d probably had COVID about 8 weeks after the fact. 🤷🏻‍♀️ #covidtoes Red mottled skin on the foot in the reticularis pattern (vasRed mottled toes
Subsequent derm papers showed actual virus particles in the small vessels in the feet of similar patients, also patterns of type 1 interferonopathy vasculitis.

There is an intriguing theory about “COVID toes.” You can get it as a result of SARS-CoV-2 infecting you.

But you can also – perhaps – get it *without* being infected, as a result of hanging out at home for long periods without shoes. It's an epidemiological puzzle. 1/
COVID toes are thought to be a known, albeit formerly rare, condition called chilblains (also known as pernio). This used to be the bane of people living in cold damp climates (think British boarding schools, or New England). #COVIDtoes 2/
Chilblains is characterized by inflammatory skin lesions in patients exposed to non-freezing weather during late winter or early spring. These lesions typically present as painful erythrocyanotic (red-blue) discoloration, often with dead skin, on the toes, fingers, or both. 3/

#PresidentialDebates should not happen without #LiveFactChecking, especially when our #despot is infamous for lies! This is a thread which attempts to dispute some of #TheHateYam's lies. The #SecondPresidentialDebate2020 begins at 24:19 on this video.

Really interesting (follows up on the case of two sets of brothers with severe COVID-19) sciencemag.org/news/2020/09/h…
We produce interferons in response to infections. These interferons “interfere” with viral replication. Coronaviruses have some protection against this because they have a replication checking enzyme (this is why coronaviruses mutate more slowly than some other viruses like flu)
Interferon is also used as an anti-viral agent (used to be a treatment for HIV and Hep C)

1/n Continuing the discussion of the #bradykinin hypothesis, from the old thread ()
2/n You'll have to be a real multiplex cycle geek to like the next couple of tweets, but I'll get back to keeping it simple after those next tweets.

From Figure 2 of the paper, elifesciences.org/articles/59177, when the renin–angiotensin system (RAS) system goes wonky, it gets complex
3/n Click on the pix to enlarge; read the paper. The aim of these overly geeky tweets is to be able to explain soon the important bits simply. The below is just an aid to understanding the above.

(1/12) The #CovidToes mystery deepens again.

It looks like they are caused by #SARSCoV2 after all, but people have been looking with the wrong test.

It seems pretty conclusive now that #COVID19 cases in children are being missed.

A short thread... Image
(2/12) First, the paper which started it all.

Doctors in Italy noticed "an epidemic of acroischemic lesions of some toes and fingers in asymptomatic children and adolescents", at the height of the epidemic but weren’t able to do testing.
(3/12) Subsequent research found that at least some children with #CovidToes test positive (by RT-PCR) for #SARSCoV2.

This suggested that the phenomenon might be a way to spot some of the children who might be asymptomatic carriers.

It is an enormous pleasure to be part of this #paper: #COVID-PIEL, the Spanish muticentric study of the #cutaneous manifestation of #COVID19 has just been published in the prestigious journal British Journal of Dermatology! 375 patients included! Conclusions here 👇 (#thread)
A new classification of #skin manifestations of #COVID19 is proposed: 1. Acral areas of erythema-oedema with some vesicles or pustules (19%); 2. Other vesicular eruptions (9%); 3 Urticarial lesions (19%); 4. Other maculopapules (47%) and 5. Livedo or necrosis (6%)
1⃣ Acral lesions, #COVIDtoes, pseudo-chiblains..., affected young patients and lasted a mean of 12,7 days to appear in the course of COVID. They were associated with less severe disease. Itchy (30%) or painful (32%).

Okay #medtwitter and #dermtwitter, you knew it was only a matter of time, didn’t you?! Let’s do this #COVID19 themed #medthread:

#COVID TOES, a #TWEETORIAL!

#MedEd #FOAMEd #dermatology #dermatologia @AADskin @AADMember @Meddermsoc @dermhospitalist
2/
First, a word of caution – this is one person’s thoughts on a new skin finding reported in a new disease that the medical community is still learning about! As such, nothing in the #tweetorial should be regarded as definite. Just wanted to share a thought process!
3/
So, who cares? #COVID19 is devastating, at times even for the young & healthy! But it’s all respiratory, right?

Well, as we're learning, there's so much more to #SARScov2, and as a #dermatologist, this is the perfect example of how the exam helps frame possible mechanisms.
