The appearance of direct-acting antiviral agents (DAAs), which specifically target HCV proteins, has provided insights into the current situation Ceruloplasmin (Cp), where its serum levels tend to be high in myocardial infarction, neoplastic and inflammatory conditions. Objective: The aim of the current work was to evaluate the prognostic value of serum ceruloplasmin in chronic hepatitis C before and after treatment by direct-acting antiviral agents and possibility of using it as a marker for hepatitis C virus treatment response.

Atopic dermatitis (AD) is a chronic psychodermatology disease with unsatisfactory pharmacotherapy. Objective The aim was to examine the effectiveness of urge surfing in patients with chronic AD. Patients and methods All patients with chronic AD who were referred to psychiatric clinic from January 1, 2018 to July 30, 2018 were included in the study. The authors used Eczema Area and Severity Index (EASI) and Dermatology Life Quality Index (DLQI) to assess the patients at baseline (T1), then at 12 weeks (T2) and lastly after 24 weeks (T3). Results The mean±SD age of the patients was 33±13.7 years, range 17–70 years, 70% were married, and 40% had completed high education. The mean±SD duration of illness was 23.1±17.8 years. The mean (EASI) score at T1 was 46. This decreased significantly to 32 at T2 (t=7.43, d.f.=0.33, P<0001). The mean DLQI at T1 was 13, decreasing significantly to 6 at T2 (t=7.98, d.f.=0.33, P<0001). A linear regression analysis was performed to assess the association between EASI and DLQI and was statistically significant (b=0.20, P=0.004). Conclusion There was a significant reduction in eczema severity and improvement in quality of life following urge surfing in resistant eczema.

Hepatitis C virus infection is associated with diabetes mellitus (DM) type II, and diabetic patients are at higher risk of acquiring HCV infection. Sweat function has been proposed to assess early autonomic neuropathy. This study aimed to evaluate small fiber neuropathy in asymptomatic HCV-related cirrhotic patients with or without DM through sweat function assessment by Sudoscan test. Three groups were involved: 47 healthy controls, 48 HCV-related cirrhotic patients without DM (group 1), and 49 HCV-related cirrhotic patients with DM type II (group 2). All participants were subjected to liver panel tests, renal function tests, cell blood counts, HbA1c, and abdominal ultrasound. Background: As physicians in a referral hospital, we observed the association between history of ... more Background: As physicians in a referral hospital, we observed the association between history of enteric fever and somatic disorders associated with low mood. At the Al-Hussein University Hospital, Cairo and the National Liver Institute Hospital, Menoufia, we receive patients from all over Egypt, including rural areas where enteric fever is endemic. Aim: Here in, 60 Egyptian patients referred to us for evaluation of different somatic disorders are reported. Methods: After extensive evaluations, the patients' symptoms were function-related. Also, their typhoid carrier states were documented, and the severity of depression using Hamilton-D (HAM-D) questionnaire was evaluated and recorded. All patients were treated with ceftriaxone, 2 gm, IV, daily for 15 days. The clinical evaluation and Hamilton score were reassessed at the end of the treatment and 6 weeks thereafter. The patients did not receive any anti-depressant nor anti-anxiety treatment during their course. Typhoid carrier was defined by documenting the history of typhoid fever that was diagnosed by culturing the Salmonella species, and not by serology, isolated from stool culture along with febrile condition, plus the absence of fever in the past 3 weeks. The Widal test was not accepted as a criterion for enrollment. Results: Patients showed clinically significant improvement in the somatic complaints, and their HAM-D score immediately post-treatment that was consolidated for 6 weeks posttreatment completion. Conclusion: In this study, the typhoid carrier was associated with the psychosomatic depression that improved by antibiotic therapy.

At our hospital, Al-Hussein University Hospital, Cairo, Egypt, we are receiving patients from all over Egypt, including rural areas where enteric fever is endemic. Here we report this series for 15 patients referred to us for evaluation of different somatic disorders. Clinical presentation: After extensive evaluations, the patients' symptoms were proved to be functional, their typhoid carrier states were documented, they were evaluated for depression using Hamilton-D questionnaire and the severity of depression was recorded. All patients were treated by ceftriaxone, 2 g, IV, daily for 15 days. Clinical evaluation and Hamilton score were reassessed at the end of treatment and 6 weeks thereafter. The patients did not receive any anti-depressant nor anti-anxiety treatment during their course. Typhoid carrier was defined by documenting the history of typhoid fever that was diagnosed by culturing the Salmonella species not by serology, plus at least one occasion of Salmonella isolates from stool culture while afebrile, plus absence of fever in the past 3 weeks. The Widal test wasn't accepted as a criterion for enrolment. Results: Patients showed clinically significant improvement in term of the presenting somatic complaints as well as their Hamilton-D score immediately post-treatment and consolidate for 6 weeks post-treatment completion. Conclusion: Typhoid carrier in our series was associated with psychosomatic depression that improved on antibiotic therapy.

While the observation that the interactions between infection and thrombosis is a well-known fact... more While the observation that the interactions between infection and thrombosis is a well-known fact, the mechanism behind this connection was not clear. Also, it was known for a long period that thrombus contains neutrophils within its skeleton. Histon an intranuclear component, was known to have a very strong microbicidal property, the question was how could it reach its target? Could the discovery of this new mechanism in neutrophil biology; namely neutrophil extracellular trap (NET), answer these issues? Hopefully it could. This review will focus on the production of NET (NETosis), its implications in different diseases, its role in understanding the connection between infection, inflammation and thrombosis, finally we will look for the potentials of targeting it, for therapeutic benefits.
