Se estudia, desde un punto de vista teorico, la transmision de oscilaciones de concentracion de m... more Se estudia, desde un punto de vista teorico, la transmision de oscilaciones de concentracion de metabolitos a traves de rutas metabolicas con cineticas tipo Michaelis-Menten. En una primera aproximacion se examinan los sistemas secuenciales y ramificados que convergen a un punto con dinamicas lineales. De aqui resultan expresiones analiticas de la relacion entre las amplitudes de salida y entrada (amortiguamiento) y de los desfases de las velocidades de cada peso de reaccion con el flujo de entrada y entre si. Se muestra que a efectos de la transmision de senales sinusoidales las enzimas michaelianas presentan aproximadamente un comportamiento lineal alrededor del fujo medio de entrada, con independencia de la amplitud de la onda. Por ello, es posible predecir las propiedades de dicha transmision a partir de sus correspondientes sistemas lineales siempre que se conozcan las constantes de velocidad de primer orden efectivas. De la misma forma, se ha demostrado que los sistemas ramificados enzimaticos con enzimas cineticamente diferentes (a efectos de la transmision de las senales sinuosidales) se pueden reducir a rutas secuenciales para un conjunto dado de actividades enzimaticas. Ademas, se ha concluido que las enzimas que mejor transmitiran una senal oscilatoria sinusoidal, seran aquellas que trabajen a baja saturacion e irreversiblemente. A study of 130 patients having presented either a transient ischemic attack or a completed stroke... more A study of 130 patients having presented either a transient ischemic attack or a completed stroke, or admitted for an asymptomatic carotid stenosis, allowed the following investigations: CT-scan, doppler, angiography, measurement of the regional cerebral blood flow (rCBF) by the study of 133Xe clearance (Novocerebrograph (R)), and measurement of the regional cerebral blood volume (rCBV) made by gamma emission tomography after autotransfusion of 99m Tc-labeled erythrocytes with an Elscint Apex 415 ECT (R) camera. Our aim was to look in which case a rCBF and/or a rCBV interhemispheric asymmetry was observed, by comparing the previous hemodynamic parameters with clinical, CT-scan and angiographic data. Although a rCBF and/or a rCBV asymmetry appeared more often among patients presenting with infarction than among those of the asymptomatic category, this does not seem specific for any clinical grade. The same statement applies to the comparison between hemodynamic parameters and CT data. The arterial lesions were classified in 4 groups: no stenosis (12 cases), uni or bilateral moderate carotid stenosis (40 cases), unilateral severe carotid stenosis or occlusion (65 cases), bilateral carotid severe stenosis or occlusion (13 cases). We observed a rCBF asymmetry among some patients in the 4 groups, although this was more frequent among the two latter groups. Nevertheless no case of a rCBV asymmetry was observed in the 2 first groups. Such an asymmetry was present in 31 percent of the patients belonging to the two latter groups. Introduction: The medical conditions shared by hospital emergency services and community-hospital... more Introduction: The medical conditions shared by hospital emergency services and community-hospital neurology clinics (CHNC) have not been described, and the quality of the medical care received in these conditions has not been evaluated in our environment. Methods: Over a 2 month period, those patients presenting at any of the seven CHNC in a Health Care Area 1 of Madrid due to previously attended medical conditions in the emergency services were systematically registered. The area neurologists of the CHNC collected administrative and clinical variables and made a judgment on the medical care (primary outcome measure) and diagnoses (secondary outcome measure) received. Results: A total of 181 patients were included (mean age: 58 years; 60% women). The inclusion rate was one patient per working day, and 31% of patients were visited out of the established quota number of patients for the clinic. The most frequent reasons for visiting the emergency room were: headache (20%), focal neurological syndrome (16%) and loss of consciousness (14%). The most frequent diagnoses at the CHNC were: primary headache (19 %), stroke (11%) and epilepsy (9 %). Emergency care was deemed correct in 56 % of patients. When the patients with intervention were compared to those with no intervention, participation of the neurology service in the emergency room was associated to a greater percentage of correct diagnoses (59% vs. 41%; p=0.019) and care (69% vs. 47%; p=0.003). Conclusions: The medical conditions shared with the emergency services represent a small but relevant proportion of the patients assisted in the CHNC. Some of these conditions (primary headaches, syncopes) should be canalized into primary health care. Others (epilepsy) require a circuit between emergency room and CNNC, but the appointment system should be adapted. The intervention of a neurologist in the emergency room raises the quality of the care.

The use of the empiric methods will allow to determine the main difficulties in the teaching of the Physical Education in school with special educational necessities and the deficiencies in the integral formation, the strategy is structured starting from the modulation of the different elements that compose it after having determined the necessities and the students' interests and the objective institutionalism. Purpose To analyze the evolution post-cardiac resynchronization therapy (CRT) in left ventricular... more Purpose To analyze the evolution post-cardiac resynchronization therapy (CRT) in left ventricular non-compaction (LVNC) cardiomyopathy (CM) patients compared to other types of CM, according to clinical and functional variables, by using gated-SPECT myocardial perfusion imaging (MPI). Methods Ninety-three patients (60 ± 11 years, 28% women) referred for pre-CRT assessment were studied and divided into three groups: 1 (non-ischemic CM with LVNC, 11 patients), 2 (ischemic CM, 28 patients), and 3 (non-ischemic CM, 53 patients). Combined assessment of perfusion and function improves diagnostic and prognostic power of gated-S... more Combined assessment of perfusion and function improves diagnostic and prognostic power of gated-SPECT in patients with coronary artery disease. The aim of this study was to investigate whether the presence of stress-induced ischemia is associated with abnormal resting left ventricular (LV) function and intraventricular dyssynchrony. Gated-SPECT myocardial perfusion imaging (MPI) at rest and 15 min post-stress was performed in 101 patients, who were divided into three groups: those with stress-induced ischemia (Group 1, n = 58), those with normal scans (Group 2, n = 28), and those with scar but no ischemia (Group 3, n = 15). More extensive perfusion defects were found in patients of Groups 1 and 3 [Summed stress score (SSS): 13 ± 8 and 21 ± 9, respectively]. In Group 2, the mean SSS was 1.5. The mean change in LV ejection fraction (LVEF at stress - LVEF at rest) was higher in Group 1 v. Group 2 patients: -5.54% ± 6.24% vs -2.46% ± 5.56%, p = 0.02. Group 3 patients also had higher values, similar to Group 1: -6.47% ± 8.82%. Patients with ischemia had almost 50% higher end-diastolic volumes than patients with normal MPI. Similarly, end-systolic volumes were almost twice as high in this group (p < 0.0001). In addition, the histogram bandwidth, a measure of intraventricular dyssynchrony, was greater in Group 1. Baseline differences in left ventricular volumes and degree of dyssynchrony are associated with inducible ischemia on stress testing in a gated-SPECT MPI. Stress-induced ischemia increases the degree of intraventricular dyssynchrony.
Quantitative hepatic angioscintigraphy was combined with duplex Doppler in order to study liver p... more Quantitative hepatic angioscintigraphy was combined with duplex Doppler in order to study liver perfusion in normal subjects and in 148 patients with liver cirrhosis. The portal component of liver perfusion determined by scintigraphy was reduced in patients with liver cirrhosis and correlated to the development of cirrhosis and to porto-hepatic gradient pressure. Duplex Doppler allowed assessment of portal blood direction. Determination of portal blood flow was possible in only a few patients: portal blood flow was increased in the first stage of cirrhosis and then decreased; hepatofugal flow was observed only in the most severe stage. Angioscintigraphy and Duplex Doppler appear to be complementary in the study and follow-up of portal hypertension.

Quantitative hepatic angioscintigraphy was combined with duplex Doppler in order to study liver p... more Quantitative hepatic angioscintigraphy was combined with duplex Doppler in order to study liver perfusion in normal subjects and in 148 patients with liver cirrhosis. The portal component of liver perfusion determined by scintigraphy was reduced in patients with liver cirrhosis and correlated to the development of cirrhosis and to porto-hepatic gradient pressure. Duplex Doppler allowed assessment of portal blood direction. Determination of portal blood flow was possible in only a few patients: portal blood flow was increased in the first stage of cirrhosis and then decreased; hepatofugal flow was observed only in the most severe stage. Angioscintigraphy and Duplex Doppler appear to be complementary in the study and follow-up of portal hypertension.
