A doença de Madelung é caracterizada por deposição múltipla e simétrica de gordura, acometendo pr... more A doença de Madelung é caracterizada por deposição múltipla e simétrica de gordura, acometendo preferencialmente a extremidade superior do corpo, com tendência à recidiva após o tratamento. Difere de outras lipomatoses pela ausência de cápsula envolvendo a lesão. O gênero masculino é o mais acometido. A etiologia parece estar relacionada a uma disfunção mitocondrial ocasionando uma lipólise deficiente. Etilismo está associado à maioria dos casos. A única abordagem terapêutica é o tratamento cirúrgico com remoção das lesões. Apresentamos o caso de um paciente do gênero masculino, etilista, submetido à cirurgia para tratamento da DM. Descritores: Lipomatose Simétrica Múltipla. Pescoço. Tomografia Computadorizada por Raios X. The Madelung's Disease is characterized by multiple and symmetric fat tissue deposition, affecting the upper body extremity, and with tendency to recurrence after the treatment. There is no capsule involving the lesions, which differs from other lipomatosis. Men are more affected. The etiology seems to be related to the mitochondrial dysfunction resulting in deficient lipolisis. Alcoholism is associated in most cases. The only possible therapeutic approach is surgery with resection of the lesions. We report the case of a man with chronic alcohol use history, submitted to surgery for treatment of the disease.

Arterial lactate as a predictor of postoperative complications in head and neck squamous cell carcinoma

In surgical patients, the use of arterial lactate to assess hypoxemia and severe inflammatory states is well-founded. However, there are few studies on its use in patients with head and neck squamous cell carcinoma. The aim of this study was to investigate whether the serum arterial lactate level on the 1st postoperative day would be a predictor of postoperative complications in head and neck squamous cell carcinoma surgeries. METHODS This is a prospective cohort, which evaluated 44 adult patients of both genders, with HNSCC, who underwent surgery associated with monobloc neck dissection as an initial treatment. Patients were divided into two groups, according to the presence or absence of postoperative complications: with complication (Clavien-Dindo II-V) and without complications (Clavien-Dindo 0-I). Student's t-test and its variants were used to compare continuous data. Pearson's or Spearman's test was used to correlate the data and p values <0.05 were considered statistically significant. RESULTS A total of 59% of the patients (n=26/44) developed postoperative complications. Serum lactate was significantly higher in the group with complications when compared to patients without complications, respectively 2.15mmoL/L (1.10-3.90) and 1.59 mmoL/L (0.70-3.44); p=0.03. The prognostic accuracy of arterial lactate was 69% (95% CI: 54%-82%; p=0.03), estimated by the ROC curve. A cut-off >1.7mmoL/L was identified, with a sensitivity of 65.38% and specificity of 66.67%. CONCLUSION Arterial lactate measured on the first postoperative day is a good predictor of postoperative complications in patients with head and neck squamous cell carcinoma.

OBJECTIVE We attempted to determine which malnutrition diagnostic variables can be used as predictors of postoperative complications in patients with head and neck squamous cell carcinoma. METHODS Forty-one patients undergoing surgery for head and neck squamous cell carcinoma were submitted to a prospective evaluation. Biochemical data, anthropometric measurements and evaluation of body composition were used in the nutritional analysis. RESULTS Twenty-two patients (53.6%) developed complications. Serum albumin measured on the first postoperative day was the only variable that significantly differed between groups. A cut-off value of 2.8 g/dL distinguished between patients with a complicated and uncomplicated postoperative course. Normalization of albumin levels occurred more frequently and more rapidly in the noncomplicated group. CONCLUSION Serum albumin measured on the first postoperative day was the only variable that was a predicter of postoperative complications after major head and neck squamous cell carcinoma surgery.

O carcinoma folicular e ainda mais raro, correspondendo a menos de 6% dos carcinomas do DTG. Relatamos um caso de carcinoma folicular de cisto do DTG em uma mulher de 70 anos. A citologia da puncao aspirativa por agulha fina pre-operatoria da lesao nao foi capaz de estabelecer o diagnostico de malignidade, sendo este revelado somente atraves do exame anatomo-patologico do especime cirurgico.

Method: A cross-sectional study was carried out from January 2017 to May 2018 in individuals with a diagnosis of head and neck cancer. The socioeconomic and clinical data were collected on a specific form from the medical record, and the quality of life evaluation was performed through the University of Washington Quality of Life Questionnaire (UW-QoL), version 4. The evaluation of anxiety was done through the Hospital Anxiety and Depression Scale (HADS). Results: In relation to topography, 43% of the cases were in the mouth, 24% in the larynx, and 16% in the oropharynx. Regarding the composite quality of life domain of UW-QoL, patients had an average of 863. Regarding anxiety, 26.6% of the patients were anxious, according to the cut-off point of the HADS. Incisional biopsy indicated a central ossifying fibroma. Conservative surgical treatment was performed, with enucleation and total curettage of the lesion. Microscopy showed proliferation of cells in a fusiform pattern, with elongated and curvilinear, sometimes oval, nuclei; eosinophilic hyaline-like material; and a low amount of calcified material showing osteoid resemblance. The patient is in a 11-month follow-up without signs of relapse.

Clinical Evolution and Epidemiologic Aspects of a Population with Oral Epidermoid Carcinoma

To analyze the clinical and radiographic features of focal osteoporotic marrow defect (FOMD) from... more To analyze the clinical and radiographic features of focal osteoporotic marrow defect (FOMD) from 2 Brazilian oral pathology services and to discuss these findings in light of the literature. Study Design: Data regarding age, sex, anatomic location, symptoms, radiographic features, size of lesion, and diagnostic hypotheses from cases of FOMD were collected. Additionally, an electronic search was undertaken in April 2018, including cases reports and cases series of FOMD. Results: Among 61,754 oral biopsies, 28 (0.04%) were FOMD. The mean age was 39.6 years and 20 (74.1%) cases occurred in females. An asymptomatic (75.0%) radiolucent (87.5%) lesion, with a mean size of 1.3 cm in the posterior mandible (77.8%) was the most common characteristic found. Fifteen cases were obtained on systematic review. The mean age was 50.1 years-with a predilection for females (73.3%). Asymptomatic (66.7%) lesions on the posterior mandible (80.0%), with a radiolucent aspect (93.3%) and a mean size of 2.3 cm were more observed. FOMDs were present in diagnostic hypotheses in 15.8% and 14.3% of multicenter cases and systematic reviews, respectively. Conclusions: The features presented in this study have more similarities than differences when compared to those reported in the literature. Support: FAPEMIG.

Objective To evaluate the QoL of the pretreatment of patients diagnosed with head and neck cancer according to the anatomical location of the tumor. Methods A descriptive, cross-sectional study was performed on a sample of 144 patients undergoing pretreatment for cancer from February 2017 to July 2019. The University of Washington QoL Questionnaire (version 4) was used to assess the QoL. The anatomical location data were obtained from medical records. The ANOVA test was used to compare the differences in QoL according to tumor location. Osteosarcomas of the jaw are rare and represent only 2-10% of all osteosarcomas. We herein presen... more Osteosarcomas of the jaw are rare and represent only 2-10% of all osteosarcomas. We herein present a rare case of an osteosarcoma exhibiting histological characteristics of fibro-osseous lesions in the lower alveolar ridge. A 58-year-old female patient presented with the complaint of gradual swelling of the right alveolar ridge of the mandible over 3 months. Radiography revealed an illdefined radiolucency, without a periosteal reaction. Microscopical analysis revealed areas of fibro-osseous lesion containing a highly cellular fibroblastic stroma with spherical, concentric lamellated ossicles and bone. In addition, the lesion exhibited typical characteristics of chondroblastic osteosarcoma with osteoid and cartilage intimately associated with anaplastic tumor cells. The diagnosis was primary osteosarcoma. The patient was submitted to surgery with wide margins and follow-up of 1 year revealed no signs of recurrence.

Methods A descriptive, cross-sectional study was performed on a sample of 144 patients undergoing pretreatment for cancer from February 2017 to July 2019. The University of Washington QoL Questionnaire (version 4) was used to assess the QoL. The anatomical location data were obtained from medical records. The ANOVA test was used to compare the differences in QoL according to tumor location. Parotid Gland Neoplasm with Extension to the Stensen Duct: A Case Report

Osteosarcomas of the jaw are rare and represent only 2-10% of all osteosarcomas. We herein presen... more Osteosarcomas of the jaw are rare and represent only 2-10% of all osteosarcomas. We herein present a rare case of an osteosarcoma exhibiting histological characteristics of fibro-osseous lesions in the lower alveolar ridge. A 58-year-old female patient presented with the complaint of gradual swelling of the right alveolar ridge of the mandible over 3 months. Radiography revealed an illdefined radiolucency, without a periosteal reaction. Microscopical analysis revealed areas of fibro-osseous lesion containing a highly cellular fibroblastic stroma with spherical, concentric lamellated ossicles and bone. In addition, the lesion exhibited typical characteristics of chondroblastic osteosarcoma with osteoid and cartilage intimately associated with anaplastic tumor cells. The diagnosis was primary osteosarcoma. The patient was submitted to surgery with wide margins and follow-up of 1 year revealed no signs of recurrence.
