I am a research physiotherapist who has recently completed a PhD in functional assessment of knee replacement patients using clinical testing, motion capture and musculoskeletal modelling. My fist post-doc looked at a shoulder intervention for impingement and biomechanical assessment of intervention techniques. I am currently working as a senior research fellow looking into patient safety issues with a primary focus on pressure ulcer prevention

Measurement of its functional range of motion is a primary indicator for rehabilitation outcomes in therapy settings. The present study was designed to assess reliability and validity of a new standardised method using a D-Flex device to assess ankle range of motion. A cohort of 20 healthy volunteers were recruited to measure the weight-bearing ankle range of motion using three assessment tools, namely, a goniometer, inclinometer and the D-Flex measurement devices. Repeated measures were performed both between and within observers for each device over a 48 h period. Performance evaluation of each device and their reliability was assessed using intra-class correlation coefficients and Bland and Altman plots. Although significant correlations ( < 0.05) were observed between devices, there were large mean differences in ankle range of motion values ranging from 4.3°-15.7°. Effect of Humidified Noninvasive Ventilation on the Development of Facial Skin Breakdown

BACKGROUND: The use of noninvasive ventilation masks is known to cause damage to facial skin tiss... more BACKGROUND: The use of noninvasive ventilation masks is known to cause damage to facial skin tissue, which affects both the efficacy of the intervention and the patient's quality of life. The use of humidification with noninvasive ventilation is a common practice, but its relative role in the development of facial pressure ulcers has not been fully studied. METHODS: A crossover cohort design was used in this study, with 15 healthy volunteers. Each volunteer randomly received both 10 cm H 2 O of CPAP with and without humidification through an oronasal mask. Skin integrity was evaluated by measuring transepidermal water loss, skin hydration, and skin pH at the bridge of the nose. Device-skin interface conditions (pressure and microclimate) were recorded at the bridge of the nose and both cheeks. The pro-inflammatory cytokine interleukin-1 was collected from the nose bridge before and after CPAP application by using a skin analysis tape. Nasal symptoms were collected by using a validated 6-point score. RESULTS: Humidified CPAP significantly increased transepidermal water loss (P < .001) and skin humidity (P .02) compared with non-humidified CPAP. There were no significant differences in skin hydration, skin pH, skin temperature, and cytokine expression between both conditions. However, there was a trend of increased median ratios of interleukin-1 concentrations in the humidified CPAP. There was a significant increase in the interface pressure at the bridge of the nose after CPAP application (P .02), with higher interface pressure values at the nose bridge compared with both left (P .002) and right (P .003) cheeks. The participants reported elevated nasal discomfort during non-humidified CPAP. CONCLUSIONS: These findings indicated that noninvasive ventilation with humidifi-cation had a potential disrupting effect on the barrier function of facial skin, associated with changes in skin microclimate and function. Further research is required to establish the cause of mask-related skin damage and to evaluate the effects of mask design, application techniques, and air flow and humidity settings.

This finding builds on previous evidence [5], which found a good relationship at low force levels. Ultrasound imaging is currently being validating across diagnostic and rehabilitation practice, this study highlights a further use of this modality for determining muscle activity and patient specific anthropometrics for musculoskeletal models.

Assessing contractile ability of the quadriceps muscle using ultrasound imaging

Quadriceps force and rectus femoris (RF) dimensions were examined to assess USI for stimating contraction level. In 15 healthy males, mean age 24.8 years, isometric quadriceps force was measured in 90 degrees knee flexion during submaximal and maximal voluntary contractions (MVCs) and at MVC in extension. Mid-thigh cross-sectional area (CSA), depth and width, and surface electromyography (EMG) of RF were recorded. Muscle width decreased curvilinearly as both force and EMG increased. At MVC, width was 3.2 +/- 0.5 cm, 25% smaller than at rest (4.4 +/- 0.7 cm), and similar to MVC in extension (-23%). CSA decreased linearly to -18% at MVC. RF thickness increased by only 10% (at 30% to MVC). Similar width changes at MVC in flexion and extension indicate the clinical potential of USI for assessing quadriceps contractility. Sensitivity is limited to assessing strength to within 25% of MVC in young males.
